PRG凝胶在兔LARS人工韧带重建前交叉韧带术后骨韧带愈合中的应用及安全性研究 被引量:2

摘要 目的:探讨富血小板凝胶(platelet-rich gel, PRG)在兔LARS(Ligament Advanced Reinforcement System, LARS)人工韧带重建前交叉韧带术后骨韧带愈合中的应用及安全性。方法:选择家兔30只作为对象,所有家兔均完成前交叉韧带动物模型建立,随机数字表分为对照组(n=15)和观察组(n=15)。对照组仅植入LARS人工韧带,观察组在对照组基础上联合PRG凝胶治疗,30个月治疗后分别从扫描电镜观察纤维组织、组织学及安全性角度对家兔效果进行评估。结果:两组家兔均完成手术治疗,且扫描电镜下均可见明显致密的纤维组织,观察组LARS人工韧带表面覆盖较多自体组织,且高放大倍率下可见人工韧带自体纤维组织紧密、均匀排列;而对照组自体纤维组织相对较少,排列缺乏规律性;两组处理30个月后,两组LARS人工韧带关节腔内游离的部分与骨隧道内部均被自体组织包绕,且多为长梭形排列,存在少许纤维组织;HE染色下观察组排列具有一定的规律性,存在少许毛细血管浸润生长,未见坏死组织;对照组排列缺乏规律性,可见少许坏死组织;两组家兔手术后切口未见感染,家兔均未出现发热症状,切口良好,膝关节无肿胀、积液;术后3周家兔均能正常负重行走,步态、步速正常。结论:将PRG凝胶用于兔LARS人工韧带重建前交叉韧带术后骨韧带愈合中能促进纤维组织诱导能力,可促进骨组织愈合,安全性较高。 Objective: To investigate the application and safety of PRG gel in the healing of anterior cruciate ligament after LARS artificial ligament reconstruction in rabbits. Methods: 30 rabbits were selected as subjects. All rabbits were completed the animal model establishment of Anterior Cruciate Ligaments. The random number table was divided into the control group(n=15) and the observation group(n=15). The control group was implanted with LARS artificial ligament only, and the observation group was treated with PRG gel on the basis of the control group. After 30 months of treatment, the efficacy of rabbits was evaluated from the perspectives of fiber tissue, histology and safety under scanning electron microscope. Results: Surgical treatment was completed in both groups of rabbits, and obvious dense fibrous tissue was observed under scanning electron microscope. The surface of LARS artificial ligament in the observation group was covered with more autologous tissue, and tight and uniform arrangement of autologous fibrous tissue of artificial ligament was observed at high magnification rate. The autologous fibrous tissue of the control group was less and the arrangement was not regular. After 30 months of treatment in the two groups, the free parts in the articular cavity of LARS artificial ligament and the internal bone tunnel of the two groups were surrounded by autologous tissues, which were mostly arranged in a long fusiform arrangement with a few fibrous tissues. The arrangement of the observation group under HE staining showed a certain regularity, with a little capillary infiltration and growth, but no necrotic tissue. No regular arrangement was observed in the control group, and a few necrotic tissues were observed. No infection was found in the incisions of the two groups of rabbits after surgery, and no fever was found in the rabbits. The incisions were good, and there was no swelling or hydrops in the knee joints. Three weeks after the operation, all the rabbits could walk with normal weight, and their gait
