关节镜下LARS人工韧带重建前交叉韧带42例 被引量:18

Arthroscopic anterior cruciate ligament reconstruction by ligament advanced reinforcement system in 42 cases

摘要 回顾性分析2006-08/2007-03解放军广州军区广州总医院骨科收治的前交叉韧带损伤患者42例,男27例,女15例。均采用LARS人工韧带进行关节镜下前交叉韧带重建。等距点建立胫骨和股骨骨道,置入韧带后游离纤维位于关节腔内,挤压螺钉固定。本组42例患者均获得随访,术后抽屈试验,Lachman试验及轴移试验全部阴性。术前IKDC评分为D级28例,C级14例;术后早期为A级35例,B级7例,差异有显著性意义(χ2=7.243,P<0.05)。术前及术后Lysholm膝关节功能评分为(66.8±7.1)分和(94.2±4.6)分,差异有显著性意义(t=10.769,P<0.05)。所有患者均未出现滑膜炎、韧带断裂、活动明显受限等并发症。术后X射线示螺钉位置正确。 A retrospective analysis was performed in 42 patients with anterior cruciate ligament injuries, who were treated with arthroscopic reconstruction using the ligament advanced reinforcement system in the Department of Orthopaedics, Guangzhou General Hospital of Guangzhou Military Area Command of Chinese PLA from August 2006 to March 2007. Bone tunnels of the femur and the tibia were prepared with an identical interval between each other. The artificial ligament was inserted into the tunnels, and then fixed with 2 interface screws after being tightened. A follow-up checkup was made in the 42 case. They were all detected to be negative for the posterior drawer test, Lachman trial and pivot shift test. The International Knee Documentation Committee scores showed 14 cases of Grade C and 28 cases of Grade D before the surgery, while 35 cases of Grade A and 7 cases of Grade B after the surgery ( x^2=7.243, p 〈 0.05). The Lysholm scores were significantly improved from a preoperative score of (66.8±7.1) to postoperative (94.2±4.6) (t=10.769, P 〈 0.05). There were no complications occurred, such as synovitis, ligament rupture and movement limitation. Postoperative X-ray outcomes displayed the screw was correctly positioned.
