摘要
目的:分析血清单核细胞趋化蛋白-1(MCP-1)、白细胞介素-6(IL-6)、C反应蛋白(CRP)水平联合检测在小儿急性上呼吸道感染诊断中的效能。方法:选取2021年12月至2022年12月该院收治的90例急性上呼吸道感染患儿进行前瞻性研究,设为观察组;并根据感染不同类型分为细菌性感染组(n=52)与病毒性感染组(n=38);另选取同期90名健康体检者,设为对照组。比较观察组与对照组、感染不同类型患者MCP-1、IL-6和CRP水平;绘制受试者工作特征(ROC)曲线,分析MCP-1、IL-6和CRP水平单项及联合检测诊断小儿急性上呼吸道感染的效能。结果:观察组MCP-1、IL-6和CRP水平均高于对照组,差异有统计学意义(P<0.05);细菌性感染组MCP-1、IL-6和CRP水平均高于病毒性感染组,差异有统计学意义(P<0.05);ROC曲线分析结果显示,MCP-1、IL-6和CRP水平单项及联合检测诊断小儿急性上呼吸道细菌性感染的曲线下面积(AUC)分别为0.772、0.679、0.807、0.972,均具有一定诊断效能,其中联合检测诊断效能最高。结论:血清MCP-1、IL-6、CRP水平联合检测诊断小儿急性上呼吸道感染的效能高于三者单项检测。
Objective:To analyze efficiency of combined detection of serum monocyte chemoattractant protein-1(MCP-1),interleukin-6(IL-6)and C-reactive protein(CRP)in diagnosis of acute upper respiratory tract infection in children.Methods:90 children with acute upper respiratory tract infection admitted to this hospital from December 2021 to December 2022 were selected for the prospective study and were set as observation group.According to the different types of infection,they were divided into bacterial infection group(n=52)and viral infection group(n=38).Another 90 healthy subjects in the same period were selected as the control group.The levels of MCP-1,IL-6 and CRP in the observation group and the control group as well as the patients with different types of infection were compared.The receiver operating characteristic(ROC)curve was drawn to analyze the efficiencies of single and combined detection of MCP-1,IL-6 and CRP in the diagnosis of acute upper respiratory tract infection in the children.Results:The levels of MCP-1,IL-6 and CRP in the observation group were higher than those in the control group,and the differences were statistically significant(P<0.05).The levels of MCP-1,IL-6 and CRP in the bacterial infection group were higher than those in the viral infection group,and the differences were statistically significant(P<0.05).The ROC curve analysis showed that the AUCs of single and combined detection of MCP-1,IL-6 and CRP levels in the diagnosis of acute upper respiratory tract bacterial infection in the children were 0.772,0.679,0.807 and 0.972,separately,all of which had certain diagnostic efficiency,and the combined detection had the highest diagnostic efficiency.Conclusions:The efficiency of combined detection of serum MCP-1,IL-6 and CRP levels in the diagnosis of acute upper respiratory tract infection is higher than that of single detection of the three.
