摘要
目的探讨白细胞(WBC)与C反应蛋白(CRP)、血清降钙素原(PCT)、白细胞介素-6(IL-6)和血清淀粉样蛋白A(SAA)联合检测在儿童上呼吸道感染性疾病中的应用价值。方法收集我院2020年10月至12月诊断为上呼吸道感染的儿童50例作为研究组,以同期健康儿童26名作为对照组,回顾性分析WBC、CRP、PCT、IL-6和SAA等指标在儿童上呼吸道感染性疾病中的表达水平,并采用受试者工作特征曲线(ROC)评价各炎性指标单独检测与联合检测时的诊断效能。结果研究组WBC、CRP、SAA、PCT和IL-6均高于对照组(P<0.01);研究组各炎症指标的升高比例不同;在儿童上呼吸道感染的诊断中,WBC、CRP、SAA、PCT、IL-6的曲线下面积(AUC)分别为0.777,0.795、0.908、0.920、0.916;WBC联合SAA的AUC最大为0.992。结论在诊断儿童上呼吸道感染时,WBC联合其他炎症指标的诊断效能高于单独检测,以WBC联合SAA的AUC最大。多指标联合检测,更利于疾病的诊断和疗效判断,值得临床推广。
Objective To investigate the application value of combined detection of white blood cells(WBC)and C-reactive protein(CRP),procalcitonin(PCT),interleukin 6(IL-6)and serum amyloid A(SAA)in children with upper respiratory tract infectious diseases.Methods Fifty children diagnosed with upper respiratory tract infection(experimental group)and 26 healthy children(healthy control group)admitted to Jincheng People's Hospital from October 2020 to December 2020 were enrolled in this study.The levels of WBC,CRP,PCT,IL-6 and SAA of these children were retrospectively analyzed.The receiver operating characteristic curve(ROC)was used to evaluate the diagnostic efficacy of each inflammatory index when tested individually and Jointly.Results The WBC,CRP,SAA,PCT and IL-6 in the study group were all higher than those in the healthy control group(P<0.01).The increase ratio of each inflammation index in the study group was different.In the diagnosis of upper respiratory tract infection in children,the area under the curve(AUC)of WBC,CRP,SAA,PCT and IL-6 were 0.777,0.795,0.908,0.920,0.916,respectively.WBC combined with SAA had the largest AUC(0.992).Conclusion The diagnostic efficiency of WBC combined with other inflammatory indicators is higher than that of independent detection in the upper respiratory tract infections in children,and the AUC of WBC combined with SAA is the largest.Given more conducive to disease diagnosis and efficacy judgment,multi-index combined detection is worthy of clinical promotion.
