膜性肾病患者抗PLA2R抗体、MCP-1与eGFR及肾组织纤维化程度的相关性 被引量:1

Correlation of anti-PLA2R antibody and MCP-1 with eGFR and degree of renal fibrosis in patients with membranous glomerulonephritis

摘要 目的 探讨膜性肾病患者血清抗M型磷脂酶A2受体(PLA2R)抗体、血清单核细胞趋化蛋白-1(MCP-1)与患者肾功能、肾组织纤维化改变的关系。方法 本研究采用回顾性研究方法,选取黄石市中医医院肾病内科2019年7月1日-2022年1月31日确诊的膜性肾病患者120例(观察组),同期因尿路结石住院治疗的患者60例作为对照组;检测二组患者的血清PLA2R、MCP-1、肾功能指标、肾组织纤维化指标,采用受试者工作曲线(ROC)分析血清PLA2R、MCP-1诊断膜性肾病患者的价值;并按照24h尿蛋白≥3.5g/d对观察组患者进行分层对比。结果 观察组的抗PLA2R抗体、MCP-1、Scr、24h尿蛋白、尿ⅣC水平显著高于对照组,差异有统计学意义(P<0.05);观察组的eGFR水平显著低于对照组,差异有统计学意义(P<0.05);抗PLA2R抗体诊断膜性肾病患者的AUC值为0.880、灵敏度为90.51%、特异度为69.39%;MCP-1诊断膜性肾病患者的AUC值为0.810、灵敏度为80.46%、特异度为73.15%;蛋白尿A组患者的抗PLA2R抗体、MCP-1均高于蛋白尿B组患者,差异有统计学意义(P<0.05);膜性肾病患者血清抗PLA2R抗体、MCP-1水平与尿ⅣC呈显著正相关性(P<0.05);膜性肾病患者血清抗PLA2R抗体、MCP-1水平与eGFR呈显著负相关性(P<0.05)。结论 膜性肾病患者血清抗PLA2R抗体、血清MCP-1水平会显著增高,血清抗PLA2R抗体、血清MCP-1水平与患者临床诊断、肾功能受损程度及肾纤维化程度有关。 Objective To investigate the relationship between serum anti-M-type phospholipase A2 receptor(PLA2R)antibody and serum monocyte chemoattractant protein-1(MCP-1)in patients with membranous glomerulonephritis,renal function and renal tissue fibrosis.Methods In this study,a retrospective research method was used to select one hundred and twenty patients with membranous glomerulonephritis diagnosed from July 1,2019 to January 31,2022 in Huangshi Traditional Chinese Medicine Hospital(observation group),and those hospitalized for urinary tract stones during the same period.60 patients were used as the control group;the serum PLA2R,MCP-1,renal function indexes and renal tissue fibrosis indexes of the two groups of patients were detected,and the receiver operating curve(ROC)was used to analyze the serum PLA2R and MCP-1 to diagnose membranous nephropathy.The value of patients with glomerulonephritis.Pa-tients in the observation group were also stratified according to 24h urinary protein 3.5 g/d.Results The lev-els of anti-PLA2R antibody,MCP-1,Scr,24-hour urine protein,and urine IVC in the observation group were significantly higher than those in the control group,with statistically significant differences(P<0.05);the eGFR level in the observation group was significantly lower than that in the control group,with statistically significant differences(P<o.05).The AUC value of anti-PLA2R antibody in diagnosing membranous ne-phropathy patients was 0.880,with a sensitivity of 90.51%and a specificity of 69.39%;the AUC value of MCP-1 in diagnosing membranous nephropathy patients was 0.810,with a sensitivity of 80.46%and a speci-ficity of 73.15%.The levels of anti-PLA2R antibody and MCP-1 in proteinuria group A were significantly higher than those in proteinuria group B,with statistically significant differences(P<0.05).There was a significant positive correlation between serum anti-PLA2R antibody and MCP-1 levels and urine IVC(P<0.05),and there was a significant negative correlation between serum anti-PLA2R antibody and MCP-1 lev
