摘要
目的观察EB病毒感染继发噬血细胞综合征(EBV-HLH)患儿外周血T淋巴细胞亚群及细胞因子水平的变化情况。方法选取EBV-HLH患儿23例(EBV-HLH组)和EBV感染相关传染性单核细胞增多症(EBV-IM)患儿29例(EBV-IM组)。采集两组患儿静脉血,使用流式细胞仪检测T淋巴细胞亚群CD3+(总T淋巴细胞)、CD4+CD3+(调节性T淋巴细胞)、CD8+CD3+(杀伤性T淋巴细胞)中的PD1+细胞(程序化死亡受体)、CD57+细胞(终末衰老T细胞)、CD69+活化细胞(刺激后活化T细胞)比例。使用荧光微珠结合流式细胞术检测外周血T淋巴细胞因子IL-2、IL-4、IL-6、IL-10、TNF-α、IFN-γ水平。结果EBV-HLH组活化的CD69+CD3+T淋巴细胞比例高于EBV-IM组(P<0.05);PD1+CD3+T淋巴细胞比例EBV-HLH组低于EBV-IM组,特别是杀伤效应功能的PD1+CD8+CD3+T淋巴细胞降低更明显(P均<0.05);两组终末衰老的CD57+CD3+T淋巴细胞比例无统计学差异(P>0.05)。EBV-HLH组IL-2、IFN-γ(促进T细胞终末分化及扩增)、IL-10(抑制T细胞增殖)均高于EBV-IM组(P均<0.05)。结论EBV-HLH患儿外周血杀伤效应功能T淋巴细胞下降,而非衰老细胞增多,提示EBV-HLH可散存在T淋巴细胞功能耗竭。
Objective To observe the changes of T lymphocyte subsets and cytokines in peripheral blood of children with Epstein-Barr virus associated hemophagocytic lymphohistiocytosis(EBV-HLH).Methods Twenty-three children with EBV-HLH(EBV-HLH group)and 29 children with EBV-infection-associated infectious mononucleosis(EBV-IM)(EBV-IM group)were selected.Venous blood samples were collected from the two groups.The proportions of PD1+cells,CD57+cells and CD69+activated cells in T lymphocyte subsets of CD3+(total T lymphocytes),CD4+CD3+(regulatory T lymphocytes)and CD8+CD3+(killer T lymphocytes)were detected by flow cytometry.The expression levels of cytokines IL-2,IL-4,IL-6,IL-10,TNF-αand IFN-γwere detected by fluorescence microbeads combined with flow cytometry.Re⁃sults The proportion of activated CD69+CD3+T lymphocytes in the EBV-HLH group was higher than that in the EBV-IM group(P<0.05).The proportion of PD1+CD3+T cells in the EBV-HLH group was lower than that in the EBV-IM group,especially the proportion of PD1+CD8+CD3+T cells with killer function was much more obvious(P<0.05).There was no significant difference in the proportion of senescent CD57+CD3+T cells between the two groups(P>0.05).The levels of IL-2,IFN-γand IL-10 in the EBV-HLH group were significantly higher than those in the EBV-IM group(all P<0.05).Con⁃clusion The killer function of T lymphocytes in peripheral blood of children with EBV-HLH decreased,not the increase of senescent cells,which was partly reflected in the exhaustion of T lymphocytes function in EBV-HLH.
