反复种植失败患者的外周血Th17/调节性T淋巴细胞的失衡情况 被引量:1

摘要 目的探讨反复种植失败(RIF)患者外周血Th17/调节性T淋巴细胞(Treg)的失衡情况。方法纳入26例RIF患者(RIF组)和27例非妊娠期且生育功能正常的女性(对照组)。于月经周期的增殖晚期,检测两组外周血单核淋巴细胞中叉头状/翅膀状螺旋转录因子P3(FOXP3)与视黄酸受体相关孤儿受体γt(RORγt)的mRNA表达量,以及CD4~+IL-17A~+Th17/CD4~+T淋巴细胞比值和CD4~+CD25~+FOXP3~+Treg/CD4~+T淋巴细胞比值;并检测两组血清转化生长因子β1(TGF-β1)、白细胞介素(IL)-10、IL-6、IL-17A、IL-23蛋白水平。结果RIF组CD4~+IL-17A~+Th17/CD4~+T淋巴细胞比值及RORγt mRNA表达量均高于对照组,而CD4~+CD25~+FOXP3~+Treg/CD4~+T淋巴细胞比值及FOXP3 mRNA表达量均低于对照组(均P<0.05)。RIF组血清TGF-β1和IL-10水平均低于对照组,IL-6、IL-17A和IL-23水平均高于对照组(均P<0.05)。结论Treg及相关细胞因子在RIF患者外周血表达降低并伴有活性减弱,而Th17及相关细胞转录因子的表达增高且活性增强;RIF患者机体存在Th17偏移,Th17/Treg失衡可能是RIF的发病机制之一。 Objective To explore the imbalance of Th17/regulatory T lymphocyte(Treg) in peripheral blood of patients with repeated implantation failure(RIF).Methods A total of 26 RIF patients were enrolled in RIF group,and 27 women with non-pregnancy and normal fertility function were enrolled in control group.In the late proliferative phase of the menstrual cycle,the mRNA expression levels of forkhead/winged-helix transcription factor P3(FOXP3) and retinoic acid receptor-related orphan receptor gamma t(RORγt) in peripheral blood mononuclear lymphocytes,as well as the CD4^(+) IL-17 A^(+) Th17/CD4^(+) T lymphocytes ratio,and CD4^(+) CD25^(+) FOXP3^(+) Treg/CD4^(+) T lymphocytes ratio were detected in both groups;furthermore,the protein levels of serum transforming growth factor β1(TGF-β1),interleukin(IL)-10,IL-16,IL-17 A,and IL-23 were detected in both groups.Results The RIF group interpreted a higher ratio of CD4^(+) IL-17 A^(+) Th17/CD4^(+) T lymphocytes,and a higher mRNA expression of RORγt,whereas a lower ratio of CD4^(+)CD25^(+)FOXP3^(+) Treg/CD4^(+) T lymphocytes,and a lower mRNA expression of FOXP3 as compared with the control group(all P<0.05).The RIF group yielded lower levels of serum TGF-β1 and IL-10,while higher levels of IL-6,IL-17 A,and IL-23 as compared with the control group(all P<0.05).Conclusion The expressions of Treg and related cytokines in peripheral blood of RIF patients are decreased and with weakened activity,while the expressions of Th17 and related cellular transcription factors are increased and with enhanced activity;moreover,there is Th17 shift in RIF patients,and Th17/Treg imbalance may be one of the pathogenesis of RIF.
