摘要
目的分析婴幼儿肺部感染与维生素D受体基因多态性相关的危险因素。方法选取2017年2月-2019年4月新乡市中心医院收治的肺部感染婴幼儿80例(感染组)及同期普通呼吸道感染婴幼儿40例(普通组)、健康婴幼儿30名(对照组),采用酶联免疫吸附试验测定三组血清25-羟基-D3[25(OH)D3]水平及维生素D(VD)缺乏率,采用聚合酶链反应-限制性长度多态性方法检测感染组、普通组VDR基因Fok I(rs2228570)、Bsm I(rs1544410及rs4334089)、Apa I(rs7975232)基因多态性,比较各位点基因型分布频率、各位点等位基因型分布频率的差异,分析VDR基因多态性与婴幼儿肺部感染的相关性。结果感染组血清25(OH)D3水平低于普通组、对照组,而VD缺乏率高于普通组、对照组(P<0.05);感染组VDR基因rs2228570基因型与等位基因分布无统计学差异,感染组VDR基因rs1544410基因型GA、等位基因A频率高于普通组(P<0.05),感染组rs4334089基因型GA、等位基因A频率高于普通组(P<0.05),感染组rs7975232基因型CA及等位基因C频率高于普通组(P<0.05);Logistic回归分析显示,VDR基因rs1544410基因型GA及等位基因A,rs4334089基因型GA,rs7975232位点等位基因C均与婴幼儿肺部感染独立相关(P<0.05)。结论婴幼儿肺部感染后其血清VD缺乏现象严重,且VDR基因rs1544410、rs4334089、rs7975232多态性与婴幼儿肺部感染密切相关。
OBJECTIVE To explore the vitamin D receptor gene polymorphisms-related risk factors and pulmonary infection in infants.METHODS Totally 80 infants with pulmonary infection(the infection group)and 40 infants with respiratory tract common infection(the common group)who were treated in Xinxiang Central Hospital from Feb 2017 to Apr 2019 were enrolled in the study,meanwhile,30 healthy infants were set as the control group.The serum 25-hydroxy-D3[25(OH)D3]level and deficiency rate of vitamin D(VD)were determined by using enzyme-linked immunosorbent assay,the gene polymorphisms of VDR genes Fok I(rs2228570),Bsm I(rs1544410 and rs4334089)and Apa I(rs7975232)of the infection group and the common group were detected by means of polymerase chain reaction-restriction fragment length polymorphism,the genotypic distribution frequencies and allele genotypic distribution frequencies in different loci were compared,and the correlation between the polymorphisms of VDR genes and the pulmonary infection in the infants was analyzed.RESULTS The serum 25(OH)D3 level of the infection group was significantly lower than that of the common group and the control group,the deficiency rate of VD of the infection group was significantly higher than that of the common group and the control group(P<0.05).There was no significant difference in VDR gene rs2228570 genotype and allele distribution in the infection group,the VDR gene rs1544410 genotype GA and allele A frequencies of the infection group were significantly higher than those of the common group(P<0.05),the rs4334089 genotype GA and allele A frequencies of the infection group were significantly higher than those of the common group(P<0.05),the rs7975232 genotype CA and allele C frequencies of the infection group were significantly higher than those of the common group(P<0.05).Logistic regression analysis indicated that the VDR gene rs1544410 genotype GA and allele A,rs4334089 genotype GA and rs7975232 locus allele C were independently associated with the pulmonary infection in the infant
