维生素D动态变化与小儿肺部感染及炎症发展程度的预测与评估价值研究

摘要 目的 探讨维生素D水平与小儿肺部感染炎症指标的相关性,并分析维生素D在小儿肺部感染的诊断与病情评估中的价值。方法 选择2022年1月—2022年12月许昌市中心医院收治的200例肺部感染患儿为研究对象,根据胸片或胸部CT检出结果的不同,分为轻度组(100例,未达重度标准、生命体征稳定)和中重度组(100例,达到中重度标准、呼吸频率>30次/min、病灶累及双侧或多肺叶)。用酶联免疫吸附法对血清25羟维生素D_(3)[25-(OH)D_(3)]进行测定,比较两组的25-(OH)D_(3)差异,探讨25-(OH)D_(3)与小儿肺部感染炎症指标的相关性及在病情预测诊断中的应用价值。结果 轻度组入院后48 h的血清25-(OH)D_(3)值高于中重度组,CRP、IL-6、TNF-α、PCT、WBC值均低于中重度组,差异有统计学意义(P<0.05)。Pearson相关性分析,25-(OH)D_(3)与CRP、IL-6、TNF-α、PCT、WBC呈负相关(r=-0.325、-0.526、-0.622、-0.725、-0.688,P<0.05)。logistic回归分析得,25-(OH)D_(3)高表达是中重度肺部感染的保护因素,CRP、IL-6、TNF-α、PCT、WBC高表达是中重度肺部感染的危险因素(P<0.05)。结论 维生素D高表达是中重度肺部感染的保护因素,血清25-(OH)D_(3)与CRP、IL-6、TNF-α、PCT、WBC有相关性。对肺部感染患儿进行血清25-(OH)D_(3)检测,能反映维生素D水平和炎症程度,能为小儿肺部感染程度临床诊断评估提供重要参考依据。 Objective To explore the correlation between vitamin D level and inflammation indicators of pediatric pulmonary infection and to analyze the value of vitamin D in diagnosis and evaluation of pediatric pulmonary infection.Methods A total of 200 children with pulmonary infection admitted to the Xuchang Central Hospital from January 2022 to December 2022 were selected as research objects.According to the results of chest radiographs or chest CT,they were divided into mild group(100 cases,not up to the standard of severe,with stable vital signs)and moderate and severe group(100 cases,reached moderate to severe standard,respiratory rate>30 times/min,lesions involving bilateral or multiple lung lobes).Serum 25 hydroxyvitamin D,[25-(OH)D_(3),] was determined by enzymed-linked immunosorbent assay,and the difference of 25-(OH)D_(3),between the two groups was compared.The correlation between 25-(OH)D_(3) and inflammatory indicators of pulmonary infection in children and its application value in disease prediction and diagnosis were discussed.Results The serum 25-(OH)D_(3),value of mild group was higher than that of moderate and severe group 48h after admission,and the values of CRP,IL-6,TNF-α,PCT and WBC were lower than those of moderate and severe group,with statistical significance(P<0.05).Pearson correlation analysis showed that 25-(OH)D_(3),was negatively correlated with CRP,IL-6,TNF-α,PCT and WBC(r=-0.325,-0.526,-0.622,-0.725,-0.688,P<0.05).logistic regression analysis showed that high expression of 25-(OH)D_(3),was a protective factor for moderate and severe lung infection,and high expression of CRP,IL-6,TNF-α,PCT and WBC was a risk factor for moderate and severe lung infection(P<0.05).Conclusion High expression of vitamin D is a protective factor for moderate and severe pulmonary infection.Serum 25-(OH)D_(3),is correlated with CRP,IL-6,TNF-α,PCT and WBC.Serum 25-(OH)D_(3),detection in children with pulmonary infection can reflect the level of vitamin D and the degree of inflammation,and provide an important refe
