伴高泌乳素血症多囊卵巢综合征患者胰岛β细胞功能及胰岛素抵抗分析 被引量:4

摘要 目的分析伴高泌乳素血症多囊卵巢综合征患者胰岛β细胞功能及胰岛素抵抗的关系。方法选取2017年1月~2018年8月我院收治的多囊卵巢综合征(PCOS)患者72例,根据其泌乳素(PRL)检查结果分成合并高泌乳素血症者作为观察组(PRL>572.43μIU/mL,36例)和单纯PCOS者作为对照组(PRL<572.43μIU/mL,36例),采集两组患者一般资料、胰岛β细胞功能及胰岛素抵抗等相关指标进行分析比较。结果两组患者血清E2、LH及FSH水平基本相似,差异均无统计学意义(P>0.05);观察组患者PRL和T水平均较对照组高,差异均有统计学意义(P<0.05);两组患者组间不同时间点糖耐量和胰岛素释放参数相比,差异均有统计学意义(P<0.05);观察组患者0、60及120min血糖水平均较对照组高,而血清胰岛素水平均较对照组低,差异均有统计学意义(P<0.05);观察组患者HOMA-IR、HOMA-β、ISI及INSAUC等指标均较对照组患者低,而△I60/△G60、DI及GLUAUC等指标均较对照组患者高,差异均有统计学意义(P<0.05)。结论伴高泌乳素血症多囊卵巢综合征患者内分泌紊乱程度相较单纯多囊卵巢综合征患者高,且胰岛素抵抗更为明显,该类患者胰岛β细胞功能可能更容易受影响,易发生功能失调。 Objective To analyze the relationship between islet β-cell function and insulin resistance in patients with polycystic ovary syndrome complicated with hyperprolactinemia. Methods 72 patients with polycystic ovary syndrome(PCOS) who were admitted and treated in our hospital from January 2017 to August 2018 were selected.According to examination results of prolactin(PRL),they were divided into the observation group(patients with PCOS complicated with hyperprolactinemia,PRL > 572.43 μIU/mL,36 cases)and the control group(patients with PCOS,PRL < 572.43μIU/mL, 36 cases).General data,islet β-cell function,insulin resistance and other relevant indexes of patients in two groups were analyzed and compared. Results Serum E2,LH and FSH levels of patients in two groups were similar,and there was no significant difference(P > 0.05).PRL and T levels of patients in the observation group were higher than those in the control group and the differences were statistically significant(P < 0.05).There were statistically significant differences in glucose tolerance and insulin release parameters between the two groups at different time points(P < 0.05).Blood glucose levels in the observation group were higher than those in the control group at 0,60min and 120min,while serum insulin levels were lower than those in the control group.Differences were statistically significant(P < 0.05).HOMA-IR,HOMA-β,ISI and INS AUC in the observation group were lower than those in the control group,while △ I60/△ G60,DI and GLU AUC were higher than those in the control group.Differences were statistically significant(P < 0.05). Conclusion The degree of endocrine disorders in patients with polycystic ovary syndrome complicated with hyperprolactinemia is higher than that in patients with polycystic ovary syndrome alone, and insulin resistance is more significant.In this type of patients,islet β-cell function may be more easily affected and susceptible to dysfunction.
