摘要 目的探讨模拟教学在住院医师规范化培训(住培)经口气管插管教学中的应用效果,为住院医师技能规范化培训提供参考及指导。方法在住院医师经口气管插管教学课程中使用高仿真模拟人系统,同时制订操作规程,进行培训教学及考核。选取2016年10月~2017年10月深圳市人民医院重症医学科的轮转住院医师(均从未参加过气管插管培训)共60人进行经口气管插管培训,分别在培训前后进行理论考试、实践技能考核,记录培训前后成功进行气管插管的平均时间和成绩,采用课后问卷调查的方式综合评估教学效果。结果培训后理论考试成绩及技能考核成绩有明显提高,与培训前相比差异有统计学意义(P<0.05),按照操作规程进行培训,实践技能考核各单项,除第1项、第6项培训前后成绩比较差异无统计学意义,其他各项培训前后成绩比较差异有统计学意义(P<0.05)。参加培训的住院医师均认为本次模拟教学可有效提高经口气管插管技能,愿意继续接受类似模拟教学,且83%的参加培训的学员表示,经过培训,充分掌握临床操作规程,有信心参与临床抢救。结论采用模拟教学的方法,按规范操作规程行经口气管插管教学培训,更接近临床实际,可有效提高住院医师临床技能操作水平,减少由于经验不足导致的处理失误。 Objective This study explored the application effect of simulated teaching in orotracheal intubation training for residents, and provided reference and guidance for clinical skills training in standardized residency training. Methods High intelligent simulation human was used in the training courses of the orotrachea intubation in the standardized residency training. Operating regulations were formulated, and trainings and assessment were carried out in the meantime. Sixty residents, who were in residency programs from Oct. 2016 to Oct. 2017 in Critical Care Department of Shenzhen people's hospital, were trained for orotracheal intubation and they had never done such training before. Theoretical tests and skill tests before and after training were carried out. Test scores and the mean successful intubation time pre-and post-training were recorded. Training effect was evaluated by questionnaire surveys. Results Scores of theoretical tests and skill tests were significantly higher after training than before (P < 0.05). The training was conducted following operating regulations and practical skills were examined by individual items. Except number one and six items, the scores of all other items were improved significantly after training. Results from the questionnaire surveys showed that residents believed that simulated teaching could improve their orotracheal intubation skills and were willing to continue such courses. 83% trained residents expressed that after training they had fully mastered the intubation procedures and were confident of participating in the clinical rescues. Conclusion Simulation teaching and training with standard orotrachea intubation procedures are closer to actual clinical practice. This method can effectively improve the clinical skill levels of residents and reduce errors in practice caused by lack of experience.
