摘要
目的探讨2型糖尿病周围神经病变(DPN)患者血清单核细胞趋化蛋白-1(monocyte chemoattractant protein,MCP-1)的表达水平及意义。方法选择我院收治的2型糖尿病伴或不伴有周围神经病变的患者120例,其中2型糖尿病伴周围神经病变患者(DPN组)60例,2型糖尿病不伴周围神经病变患者(NDPN组)60例,另选取同期于本院体检的健康人群(NC组)40例,采用酶联免疫吸附法(ELISA)测定患者MCP-1的水平,根据多伦多系统评分(TCSS)将DPN组进一步分为轻、中和重度3个亚组。分析MCP-1与各临床指标的关系,并对DPN的危险因素进行logistic回归分析。结果NDPN组MCP-1水平明显高于NC组(P<0.05),而DPN组MCP-1水平高于NDPN组(P<0.05)。Pearson相关分析提示MCP-1表达水平与糖化血红蛋白(HbA1c)、HOMA-IR、肿瘤坏死因子α(TNF-α)、TCSS、丙二醛(MDA)和白细胞介素6(IL-6)呈正相关,与超氧化物歧化酶(SOD)呈负相关。多元逐步回归分析显示MCP-1是DPN发生发展的危险因素,与神经病变的严重程度有关(P<0.05)。结论DPN患者的血清MCP-1水平明显升高,与病情严重程度相关,提示MCP-1可能在DPN发生发展中发挥了重要作用。
Objective To explore the expression and clinical significance of serum MCP-1 levers in patients with type 2 diabetic peripheral neuropathy(DPN).Methods One hundred and twenty patients with type 2 DPN or NDPN admitted in the hospital were selected,including 60 patients with type 2 DPN and 60 patients with type 2 NDPN,40 healthy people(NC group)receiving physical examinations in the hospital were separately selected,and MCP-I level of patients was measured by enzyme linked immunosorbent assay(ELISA).The relations between MCP-1 and clinical indexes were analyzed,and the risk factors of DPN were analyzed by logistic regression.The patients in DPN group were further divided into three subgroups including mild group,moderate group and severe group based on Toronto clinical scoring system(TCSS).Results The level of MCP-1 in NDPN group was significantly higher than that in NC group(P<0.05),and the level of MCP-1 in DPN group was higher than such index in NDPN group(P<0.05).It was found in Pearson correlation analysis that the expression level of MCP-1 was positively correlated with glycated hemoglobin Ac(HbA1 c),HOMA-IR,tumor necrosis factor A(TNF-α),TCSS,malondialdehyde(MDA)and interleukin 6(1 l-6)and negatively correlated with superoxide dismutase(SOD).Multiple stepwise regression analysis showed that MCP-1 was a risk factor for the development and progression of DPN,and it was related to the severity of neuropathy(P<0.05).Conclusion Significantly elevated MCP-1 level of serums in DPN patients is related to the severity of the disease,suggesting that MCP-1 may play an important role in the development and progression of DPN.
