IL-10-592和IL-4-589基因多态性在慢性HBV/HCV感染者肝脏炎症中的交互作用 被引量:1

Interaction between IL-10-592 and IL-4-589 gene polymorphisms in the liver inflammation in patients with chronic HBV/HCV infection

摘要 目的 探讨白介素-10(interleukin-10,IL-10)-592位点和白介素-4(interleukin-4,IL-4)-589位点的基因多态性在慢性乙型肝炎病毒(Hepatitis B virus, HBV)和/或丙型肝炎病毒(Hepatitis C virus, HCV)感染者肝脏炎症中的交互作用。方法 对20世纪80年代后期河北某农村单采血浆还输血细胞献血员中的HBV和/或HCV感染者进行分析,用多因子降维法(multifactor dimensionality reduction method, MDR)分析IL-10-592和IL-4-589基因多态性在慢性HBV/HCV感染者肝脏炎症中有无交互作用,用比值比(odds ratio,OR)和95%置信区间(confidence interval, 95%CI)评价肝脏炎症风险,交互作用大小用交互作用指数(synergy index,SI)、交互作用归因比(attributable proportion of interaction,API)、交互作用超额相对危险度(relative excess risk of interaction,RERI)及95%CI评价。结果 (1)MDR分析显示IL-10-592和IL-4-589基因位点对慢性HBV/HCV感染者肝脏炎症有明显的交互作用(检验准确度=0.711 2,交叉验证一致性=10/10,P<0.01)。(2)单独IL-10-592 AC基因型、单独IL-4-589 CC/CT基因型对慢性HBV/HCV感染者肝脏炎症均未发现明显作用,OR值及95%CI分别为0.98(0.23~4.30)(χ^(2)=0.871,P>0.05)、1.79(0.52~6.15)(χ^(2)=0,P>0.05)。(3)IL-4-589 CC/CT和IL-10-592 AC基因型共存时对HBV/HCV慢性感染者肝脏炎症的OR值及95%CI为5.47(1.93~15.49)(χ^(2)=20.310,P<0.01),SI为5.81,API为67.64%,RERI为3.70(2.55~5.37)。结论 IL-10-592 AC和IL-4-589 CC/CT基因型共存对慢性HBV/HCV感染者的肝脏炎症有明显的交互作用。 Objective To investigate the interaction of interleukin-10(IL-10)-592T/C and interleukin-4(IL-4)-589T/C polymorphisms on liver inflammation in patients with chronic Hepatitis B virus(HBV)and Hepatitis C virus(HCV)infection.Methods To analyze the prevalence of HBV and/or HCV infection among single plasma blood donors in a rural area of Hebei Province in the late 1980s.Multi-factor dimensionality reduction method(MDR)was used to analyze whether there was an interaction between IL-10-592 and IL-4-589 gene polymorphisms in liver inflammation in chronic HBV/HCV infection.The odds ratio(OR)and 95%confidence interval(95%CI)were used to evaluate the risk of liver inflammation.The magnitude of interaction was evaluated by interaction index(SI),attributable proportion of interaction(API),relative excess risk of interaction(RERI)and 95%CI.Results①MDR analysis showed that IL-10-592 and IL-4-589 single nucleotide polymorphisms loci had a significant interaction on liver inflammation in chronic HBV/HCV infection(test accuracy=0.7112,cross validation consistency=10/10,P<0.01).②IL-10-592 AC genotype and IL-4-589 CC/CT genotype had no significant effect on liver inflammation in patients with chronic HBV/HCV infection,and the 0OR and 95%CI were 0.98(0.23-4.30)(x^(2)=0.871,P>0.05),1.79(0.52-6.15)(x^(2)=0,P>0.05).The IL-4-589 CC/CT and IL-10-592 AC genotypes co-existed,the OR and 95%CI was 5.47(1.93-15.49)(x^(2)=20.310,P<0.01),SI was 5.81,API was 67.64%,and RERI was 3.70(2.55-5.37).Conclusion The coexistence of IL-10-592 AC and IL-4-589 CC/CT genotypes has a significant interaction on liver inflammation in chronic HBV/HCV infected individuals.
