摘要
目的了解深圳地区类风湿关节炎(Hieumatord arthritis,RA)患者IL-17A基因rs2275913位点多态性与血清中IL-17A,IL-38水平表达的相关性。方法选择103例RA患者作为RA组,另选取116例非RA健康人群作为对照组。分别检测IL-17A,IL-38及类风湿因子(rheumatoid factors,RF)水平,同时对IL-17A基因rs2275913位点多态性进行分析。结果RA组IL-17A和RF水平(54.06±1.87 pg/ml和29.65±4.02 IU/ml)明显高于对照组(33.29±1.42 pg/ml和5.27±1.38 IU/ml),而IL-38水平(40.92±8.67 pg/ml)明显低于对照组(53.78±12.04 pg/ml),两组差异有统计学意义(t=7.2065,18.2972,5.0197,均P<0.05)。RA组随着病情活动度的加重IL-17A和RF水平明显升高,而IL-38水平明显降低,不同活动度之间差异均有统计学意义(F=12.5271,7.9351,9.2702,均P<0.05)。RA组IL-17A基因rs2275913位点AA基因型和A等位基因检出率(47.57%和63.11%)明显高于对照组(15.52%和30.60%),两组差异有统计学意义(χ^(2)=5.9256,4.0272,均P<0.05)。RA组AA基因型IL-17A水平比AG和GG基因型明显升高,而IL-38水平明显降低,不同基因型之间差异有统计学意义(F=7.0625,5.1873,P均<0.05),但不同基因型RF水平差异无统计学意义(F=1.0961,P=0.1205)。经Spearman相关性分析,RA组IL-17A水平与RF水平呈正相关,而与IL-38水平呈负相关(r=0.5012,-0.6924,均P<0.05)。结论RA组IL-17A水平明显升高,且随病情活动度加重而升高。同时IL-17A基因rs2275913位点多态性与深圳地区RA发病的易感性相关,其中AA基因型可能是RA发病的易感危险基因之一。
Objective To investigate the correlation between rs2275913 polymorphism of IL-17A gene and the expression of IL-17A and IL-38 in serum of patients with rheumatoid arthritis in Shenzhen Area.Methods 103 patients with RA were selected as the RA group,and another 116 patients without RA were selected as the control group.The levels of IL-17A,IL-38 and RF were detected respectively,and the polymorphism of IL-17A gene rs2275913 was analyzed.Results The levels of IL-17A and RF(54.06±1.87pg/ml and 29.65±4.02IU/ml)in RA group were significantly higher than(33.29±1.42pg/ml and 5.27±1.38IU/ml)in control group,while the levels of IL-38(40.92±8.67pg/ml)were significantly lower than(53.78±12.04pg/ml)in control group,with statistical significance(t=7.2065,18.2972,5.0197,all P<0.05).The levels of IL-17A and RF increased with the increase of disease activity in RA group,while the levels of IL-38 were significantly decreased(F=12.5271,7.9351,9.2702,all P<0.05).The detection rate of AA genotype and A allele at rs2275913 of IL-17A gene in RA group(47.57%and 63.11%)was significantly higher than in control group(15.52% and 30.60%),and the difference was statistically significant(χ^(2)=5.9256,4.0272,all P<0.05).The level of IL-17A in AA genotype in RA group was significantly higher than that in AG and GG genotype,while the level of IL-38 was significantly lower,and there was statistically significant difference among different genotypes(F=7.0625,5.1873,all P<0.05),but there was no statistically significant difference in the level of RF among different genotypes(F=1.0961,P=0.1205).Spearman correlation analysis showed that the level of IL-17A in RA patients was positively correlated with the level of RF,but negatively correlated with the level of IL-38(r=0.5012,-0.6924,all P<0.05).Conclusion The level of IL-17A increased significantly in RA patients,and increased with the severity of RA disease activity.At the same time,rs2275913 polymorphism of IL-17A gene was related to the susceptibility to RA in Shenzhen Area,and AA genotype
