新规范化培训护士心理一致感的中介与调节效应研究 被引量:1

摘要 目的分析新入规范化培训护士转型冲击与去人格化的关系,并探索心理一致感在两者间的中介和调节作用。方法本研究于2022年8-9月,使用一般情况表、规范化培训护士转型冲击评价量表、去人格化和心理一致感量表对3000名新规范化培训护士进行调查。结果转型冲击对新规范化培训护士的去人格化有显著的预测作用(B=0.478,P<0.001),心理一致感在规范化培训护士转型冲击与去人格化之间起到部分中介作用,Bootstrap检验结果显示:心理一致感在转型冲击与去人格化间的中介效应为0.09(95%CI:0.023~0.151),占总效应21.9%;心理一致感对转型冲击与去人格化间的关系有调节作用,事后分析表明,低心理一致感水平下在转型冲击与去人格化间的中介效应成立(95%CI:0.025~0.148),高心理一致感的情况下在转型冲击与去人格化间的中介效应不存在(95%CI:-0.12~0.14)。结论转型冲击能够正向预测新规范化培训护士的去人格化水平,但心理一致感在二者之间起到部分中介作用;同时,随着心理一致感水平的提高,转型冲击与去人格化水平间的正向关系将显著减弱。因此,基于心理一致感干预来缓解和降低转型冲击带来的去人格化。 Objective To analyze the relationship between transition shock and depersonalization of newly registerednurses,and to explore the mediation and moderating roles of sense of coherence between them.Methods From August to September 2020,a total of 3000 newly registered nurses were investigated with the general information table,the Transition Shock of Newly Graduated Nurses Scale,the Depersonalization Scale,and the Sense of Coherence Scale.Results The results showed transition shock had a significant predictive effect on newly registered nurses'depersonalisation(B=0.478,P<0.001),and sense of coherence played a partial mediating role between transition shock and depersonalization in newly registered nurses.In addition,the results of Bootstrap test showed that the sense of coherence mediated the relationship between transition shock and depersonalization whose effect was 0.091(95%CI was 0.023-0.151),accounting for 21.9%of the total effect.The sense of coherence had a significant moderating effect on the relationship between transition shock and depersonalization,and post-hoc analysis indicted that low sense of coherence had a mediating effect between transition shock and depersonalization(95%CI was 0.025-0.148),while high sense of coherence had no mediating effect between transition shock and depersonalization(95%CI was from-0.12 to 0.14).Conclusion Transitionshock can positively predict the level of newly registered nurses'depersonalization,but the sense of coherence partially mediates this effect.Moreover,with the improvement of nurses'sense of coherence,thepositive relationship between them will be significantly weakened.Therefore,interventions based on the sense of coherenceshould be used to relieve and reduce the depersonalizationcaused by transition shock in newly registered nurses.
