高通量血液透析联合血液灌流对尿毒症患者钙磷代谢和AGEs、ICAM-1水平的影响 被引量:2

Effect of high flux hemodialysis combined with hemoperfusion on calcium and phosphorus metabolism,AGEs and ICAM-1 levels in patients with uremia

摘要 目的分析高通量血液透析联合血液灌流治疗尿毒症的临床效果。方法抽取2018年2月至2020年10月河南大学第一附属医院尿毒症患者92例,按随机数表法分为联合组与透析组,每组46例。透析组采用高通量血液透析,联合组在透析组基础上采用血液灌流。对比两组疗效和治疗前后肾功能[尿素氮(BUN)、β2微球蛋白(β2-MG)]、钙磷代谢(血钙、血磷水平)、免疫功能(T淋巴细胞亚群CD3^(+)、CD8^(+))、血管内皮功能[晚期糖基化终末产物(AGEs)、细胞间黏附分子-1(ICAM-1)]。结果联合组总有效率(86.96%,40/46)高于透析组(67.39%,31/46),P<0.05;治疗后,联合组BUN、β2-MG水平低于透析组(P<0.05);治疗后,联合组血钙水平高于透析组,血磷水平较低于透析组(P<0.05);治疗后,联合组CD3^(+)水平高于透析组,CD8^(+)水平低于透析组(P<0.05);治疗后,联合组AGEs、ICAM-1水平低于透析组(P<0.05)。结论高通量血液透析联合血液灌流治疗尿毒症效果确切,可调节钙磷代谢和免疫状态,改善肾功能和血管内皮功能。 Objective To analyze the clinical effect of high flux hemodialysis combined with hemoperfusion in the treatment of uremia.Methods A total of 92 patients with uremia treated in First Affiliated Hospital of Henan University from February 2018 to October 2020 were selected,and they were divided into combined group and dialysis group by random number table method,with 46 case in group.The dialysis group was treated by high flux hemodialysis,and the combined group was treated by hemoperfusion based on the treatment of the dialysis group.The curative effect,renal function assessed by blood urea nitrogen(BUN)andβ2 microglobulin(β2-MG),calcium and phosphorus metabolism(blood calcium and phosphorus),immune function assessed by T lymphocyte subsets cluster of differentiation 3+(CD3^(+))and cluster of differentiation 8+(CD8^(+)),vascular endothelial function assessed by advanced glycation end products(AGEs)and intercellular cell adhesion molecule-1(ICAM-1)before and after treatment were compared between the two groups.Results The total effective rate of the combined group(86.96%,40/46)was higher than that of the dialysis group(67.39%,31/46),P<0.05.After treatment,the levels of BUN andβ2-MG of the combined group were lower than those of the dialysis group(P<0.05).After treatment,the serum calcium of the combined group was higher than that of the dialysis group,and the blood phosphorus was lower than that of the dialysis group(P<0.05).After treatment,the level of CD3^(+)of the combined group was higher than that of the dialysis group,and the level of CD8^(+)was lower than that of the dialysis group(P<0.05).After treatment,the levels of AGEs and ICAM-1 of the combined group were lower than those of the dialysis group(P<0.05).Conclusions High flux hemodialysis combined with hemoperfusion is effective in the treatment of uremia,which can regulate calcium and phosphorus metabolism and immune status,and improve renal function and vascular endothelial function.
