摘要
目的探讨microRNA-146a(miR-146a)、Toll样受体-4(TLR-4)、可溶性人髓系细胞触发受体1(sTREM-1)在肺癌术后肺部感染患者中的表达及预测价值。方法选取2019年6月至2020年12月在该院行肺癌手术治疗的124例肺癌患者作为研究对象,依据术后患者是否发生肺部感染分为感染组和未感染组,每组62例。采用实时荧光定量PCR检测miR-146a的表达情况,采用流式细胞术检测TLR-4水平,采用酶联免疫吸附试验测定sTREM-1水平,采用肺部感染评分(CPIS)、急性生理学及慢性健康状况评分(APACHEⅡ评分)评估两组肺部感染及健康生理状态,采用受试者工作特征(ROC)曲线分析miR-146a、TLR-4、sTREM-1对肺癌术后患者肺部感染的预测价值。结果感染组miR-146a、TLR-4、sTREM-1水平及CPIS、APACHEⅡ评分高于未感染组,差异均有统计学意义(P<0.05);术后1 d及术后3 d miR-146a、TLR-4、sTREM-1水平高于术前(P<0.05),术后3 d miR-146a、TLR-4、sTREM-1水平低于术后1 d(P<0.05)。miR-146a、TLR-4、sTREM-水平均与CPIS、APACHEⅡ评分呈正相关(P<0.05)。ROC曲线分析显示,miR-146a、TLR-4、sTREM-1三项联合预测肺癌术后肺部感染的曲线下面积(AUC)高于单项指标检测(P<0.05)。结论血清miR-146a、TLR-4、sTREM-1表达水平与肺癌患者术后易发生肺部感染相关,早期联合检测血清miR-146a、TLR-4、sTREM-1对预测患者肺癌术后发生肺部感染有一定临床价值。
Objective To investigate the expression and predictive value of microRNA-146 a(miR-146 a),Toll-like receptor-4(TLR-4),and soluble triggering receptor expressed on myeloid cell 1(sTREM-1)in patients with pulmonary infection after lung cancer surgery.Methods From June 2019 to December 2020,124 patients with lung cancer who underwent surgical treatment in our hospital were selected as the research objects.According to the occurrence of postoperative lung infection,the patients were divided into infection group and non-infection group,with 62 patients in each group.The expression of miR-146 a was detected by real-time fluorescence quantitative PCR,the TLR-4 level was detected by flow cytometry,and the STREM-1 level was determined by enzyme-linked immunosorbent assay.Pulmonary infection score(CPIS)and acute physiology and chronic health evaluationⅡ(APACHEⅡscores)were used to evaluate pulmonary infection and healthy physiological status of the two groups.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of miR-146 a,TLR-4 and sTREM-1 for pulmonary infection in patients after lung cancer surgery.Results The levels of miR-146 a,TLR-4,STREM-1,CPIS and APACHEⅡscores in the infected group were higher than those in the non-infected group,with statistical significance(P<0.05).The levels of miR-146 a,TLR-4 and sTREM-1 on postoperative day 1 and 3 were higher than those before surgery(P<0.05),and the levels of miR-146 a,TLR-4 and sTREM-1 on postoperative day 3 were lower than those on postoperative day 1(P<0.05).MiR-146 a,TLR-4 and sTREM-1 positively correlated with CPIS and APACHEⅡscores(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of miR-146 a,TLR-4 and sTREM-1 combined in predicting pulmonary infection after lung cancer surgery was higher than that of single indicator detection(P<0.05).Conclusion The expression levels of serum miR-146 a,TLR-4,and sTREM-1 are correlated with postoperative pulmonary infection in patients with lung cancer.Early combined detection
