摘要
目的探讨血清降钙素原、内毒素、可溶性人髓系细胞触发受体-1(sTREM-1)在肺癌术后并发细菌性肺部感染中的预测价值。方法依据是否发生术后细菌性肺部感染,将210肺癌术后患者分为感染组(n=70)和未感染组(n=140)。比较两组患者血清降钙素原、内毒素、sTREM-1水平;采用受试者工作特征(ROC)曲线和曲线下面积(AUC)评估上述指标对肺癌术后患者细菌性肺部感染的预测价值。结果术前,两组患者血清降钙素原、内毒素、sTREM-1水平比较,差异均无统计学意义(P﹥0.05)。术后24 h,两组患者血清降钙素原、内毒素、sTREM-1水平均明显高于本组术前;且感染组患者术后24 h血清降钙素原、内毒素、sTREM-1水平明显高于未感染组患者,差异均有统计学意义(P﹤0.05)。不同细菌感染类型、TNM分期和病理类型肺癌术后细菌性感染患者术后24 h血清降钙素原、内毒素、sTREM-1水平比较,差异均无统计学意义(P﹥0.05)。术后24 h,血清降钙素原、内毒素、sTREM-1预测肺癌患者术后并发细菌性肺部感染的AUC分别为0.882、0.941和0.767。结论血清降钙素原、内毒素、sTREM-1预测肺癌患者术后并发细菌性肺部感染有一定的应用价值,值得临床进一步研究。
Objective To investigate the serum procalcitonin,endotoxin and soluble triggering receptor expressed on myeloid cell-1(sTREM-1)in pulmonary bacterial infection after lung cancer surgery.Method 210 patients after lung cancer surgery were divided into infection group(n=70)and non-infection group(n=140)according to pulmonary bacterial infection.The serum procalcitonin,endotoxin and sTREM-1 of the two groups were compared and the predictive value of paramaters of interest was assessed with receiver operating characteristic(ROC)curve and area under curve(AUC).Result There were no significant differences for serum procalcitonin,endotoxin,and sTREM-1 in the two groups before surgery(P>0.05)versus significant higher levels 24 hours after surgery than those before surgery in both groups(P<0.05),and significantly higher serum procalcitonin,endotoxin,and sTREM-1 in infection group than those in the non-infection group(P<0.05).There were no significant differences in serum procalcitonin,endotoxin and sTREM-124 hours after surgery for pulmonary bacterial infection patients with different types of bacterial infection,TNM stages,and pathological type(P>0.05).The AUC of serum procalcitonin,endotoxin and STREM-124 hours after surgery in precdicting pulmonary bacterial lung infection was 0.882,0.941 and 0.767,respectively.Conclusion Serum procalcitonin,endotoxin and sTREM-1 have certain application value in predicting postoperative pulmonary bacterial infection in patients with lung cancer,and warrants future clinical investigation.
