摘要
目的分析可溶性细胞间黏附分子1(sICAM-1)、可溶性髓系细胞触发受体1(sTREM-1)联合神经肽Y(NPY)检测在小儿重症肺炎诊断、病情监测中的价值。方法病例对照研究。收集联勤保障部队第九〇九医院2017年12月至2020年12月收治的115例肺炎患儿的临床资料。重症肺炎64例(重症组),普通肺炎51例(普通组);另同期选取在本院进行健康体检的55名儿童作为对照组。比较不同人群sICAM-1、sTREM-1及NPY水平及不同急性生理和慢性健康状况评分(APACHEⅡ)重症肺炎患儿上述因子水平,并分析上述因子水平与APACHEⅡ评分相关性及对小儿重症肺炎的预测价值。结果重症组与普通组sICAM-1、sTREM-1及NPY水平均显著高于对照组(P值均<0.05),且上述因子水平以重症组最高。高、中危组sICAM-1、sTREM-1及NPY水平均显著高于低危组(P值均<0.05),上述因子水平以高危组最高。相关性分析结果显示,sICAM-1、sTREM-1及NPY水平与APACHEⅡ评分均呈正相关(r值分别为0.386、0.448、0.427,P值均<0.05)。受试者工作特征曲线分析结果显示,sICAM-1、sTREM-1、NPY曲线下面积(AUC)分别为0.787、0.841、0.709,以sTREM-1的AUC最大,但三者联合检测的AUC更高,为0.863。结论血清sICAM-1、sTREM-1及NPY表达水平与小儿重症肺炎病情严重程度相关,可作为诊断疾病、监测患儿病情的重要指标。
Objective To analyze the value of the detection of soluble intercellular adhesion molecule-1(sICAM-1)and soluble triggering receptor expressed on myeloid cells-1(sTREM-1)combined with Neuropeptide Y(NPY)in diagnosis and monitoring of severe pneumonia in children.Methods This was a case-control study.The clinical data of 115 pneumonia children admitted to the 909th Hospital of PLA Joint Logistics Support Force from December 2017 to December 2020 were collected,those patients were grouped:severe pneumonia(n=64,severe group)and common pneumonia(n=51,normal group),and 55 children with concurrent physical examination in our hospital as control group.The aim was to compare the levels of sICAM-1,sTREM-1 and NPY in groups,acute physiology and chronic health evaluation Ⅱ(APACHEⅡ)was used to evaluate the above factors in children with severe pneumonia.The correlation between the above factors levels and APACHEⅡ score,and its value in predicting severe pneumonia in children were analyzed.Results The levels of sICAM-1,sTREM-1 and NPY in severe group and normal group were significantly higher than those in control group(all P<0.05),which were the highest in severe group.The levels of sICAM-1,sTREM-1 and NPY in high-risk group and intermediate-risk group were obviously higher than those in low-risk group(all P<0.05),which were the highest in high-risk group.The results of correlation analysis showed that the levels of sICAM-1,sTREM-1 and NPY were positively correlated with APACHEⅡ score(r=0.386,0.448,0.427,respectively,all P<0.05).The receiver operating characteristic curve(ROC curve)showed that the areas under curve(AUC)of sICAM-1,sTREM-1,NPY and the combination of the above threes were 0.787,0.841,0.709,and 0.863,respectively,AUC of sTREM-1 was the largest,AUC of the combination of the above threes(0.863)was the highest.Conclusions For children with severe pneumonia,the levels of serum sICAM-1,sTREM-1 and NPY are associated with the severity,which is capable of being important indicators to assess and monitor the sev
