摘要
目的:探讨不同剂量氯吡格雷治疗对急性脑梗死患者神经功能及预后的影响。方法:选取2019年3月—2021年3月我院收治的100例急性脑梗死患者作为研究对象。采用随机数字表法分为研究组、对照组,每组50例。对照组给予75 mg氯吡格雷进行治疗,研究组给予150 mg氯吡格雷进行治疗,比较两组抗血小板治疗效果、神经功能评分、血流动力学指标和神经功能恶化、再发脑梗死及出血等不良反应。结果:研究组血小板计数和血小板聚集率低于对照组(P<0.05);治疗后30 d、90 d,研究组NIHSS评分低于对照组(P<0.05);研究组PSV、EDV水平高于对照组,RI水平低于对照组(P<0.05);研究组神经功能恶化、再发脑梗死发生率低于对照组(P<0.05);但两组出血发生率比较,差异无统计学意义(P>0.05)。结论:150 mg氯吡格雷对抗血小板效果更好,能够促进神经功能的早日恢复,治疗效果好。
Objective:To study the effect of different doses of clopidogrel on neurological function and prognosis in patients with acute cerebral infarction.Methods:A total of 100 patients with acute cerebral infarction admitted to our hospital from March 2019 to March 2021 were selected as subjects.They were randomly divided into study group and control group,50cases in each group.The control group was treated with 75 mg clopidogrel,and the study group was treated with 150 mg clopidogrel.The anti-platelet treatment effect,neurological function score,hemodynamic indexes,neurological deterioration,recurrent cerebral infarction and bleeding were compared between the two groups.Results:The platelet count and platelet aggregation rate in the study group were lower than those in the control group(P<0.05).The NIHSS score of the study group was lower than that of the control group at 30 and 90 days after treatment(P<0.05).PSV and EDV levels in the study group were higher than those in the control group,but RI level was lower than that in the control group(P<0.05).The incidence of neurological deterioration and recurrent cerebral infarction in the study group was lower than that in the control group(P<0.05).However,there was no significant difference in the incidence of bleeding between the two groups(P>0.05).Conclusion:150 mg clopidogrel has better anti-platelet effect,can promote the early recovery of neurological function,and has a good therapeutic effect.
