经颅彩色多普勒超声诊断大脑中动脉夹层一例分析 被引量:1

摘要 大脑中动脉夹层在临床上比较少见,早期诊治是患者避免卒中和提高生活质量的关键。该例大脑中动脉夹层患者,首先由经颅彩色多普勒超声(TCCS)诊断,并经高分辨率磁共振血管成像证实。该例大脑中动脉夹层在TCCS中表现为“螺旋双腔样”血流信号,频谱形态呈高、低频谱同平面不同位置不同混叠显示,予抗凝、降脂治疗3个月后复查TCCS未见明显改变。因此,对于大脑中动脉TCCS表现为“螺旋双腔样”血流信号时,需排除大脑中动脉夹层的诊断。 Middle cerebral artery dissection is rare,and early diagnosis and treatment is the key of avoiding stroke and improving quality of patients life.The case was firstly diagnosed by transcranial color-coded Doppler ultrasound(TCCS)and confirmed by high-resolution magnetic resonance angiography(HRMRA).This study reported a case of middle cerebral artery dissection.It was firstly showed in TCCS“spiral double lumen”blood flow,with high and low frequency spectrum morphology at different positions on the same plane.The patient received anticoagulant and lipid-lowering therapy,and no significant change was found in TCCS at 3-month follow-up.Therefore,middle cerebral artery dissection should be excluded when middle cerebral artery presents“spiral double lumen”blood flow in TCCS.
