血NLR、PLR和RDW联合检测对儿童脓毒血症的诊断价值 被引量:6

Diagnostic value of combined detection of NLR,PLR and RDW in children with sepsis

摘要 目的探讨血中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)、红细胞分布宽度(RDW)联合检测对儿童脓毒血症的诊断价值。方法选取脓毒血症患儿80例(脓毒血症组)和感染性疾病患儿80例(非脓毒血症组),入院后2 h检测WBC、RDW、血小板计数、中性粒细胞计数、淋巴细胞计数和降钙素原(PCT),计算PLR和NLR。采用ROC曲线分析PCT、NLR、PLR及RDW对儿童脓毒血症的诊断价值;采用logistic回归分析建立NLR、PLR及RDW联合诊断模型,分析联合诊断模型对儿童脓毒血症的诊断价值。结果脓毒血症组患儿PCT、NLR、PLR及RDW均高于非脓毒血症组(P<0.01)。PCT诊断儿童脓毒血症的AUC为0.813[95%CI(0.744~0.881)],当取截断值为0.64 ng/mL时,诊断儿童脓毒血症的灵敏度为82.5%,特异度为72.5%。NLR、PLR及RDW联合诊断模型诊断儿童脓毒血症的AUC为0.908[95%CI(0.858~0.958)],当取截断值为0.401时,诊断儿童脓毒血症的灵敏度和特异度分别为90.0%和85.0%。结论NLR、PLR和RDW联合检测对儿童脓毒血症有较高的诊断价值。 Objective To investigate the diagnostic value of combined detection of the ratio of neutrophil to lymphocyte(NLR),ratio of platelet to lymphocyte(PLR) and red blood cell distribution width(RDW) in the children with sepsis.Methods The WBC,RDW,platelet, neutrophil, lymphocyte and procalcitonin(PCT) were detected in 80 children with sepsis(group A) and 80 children with infectious diseases(group B) 2 hours after admission.The PLR and NLR were calculated.The ROC curve was used to analyze the diagnostic value of PCT,NLR,PLR and RDW in the children with sepsis.The combined diagnostic model was established by logistic regression analysis.The diagnostic value of combined diagnostic model in the children with sepsis was analyzed.Results The PCT,NLR,PLR and RDW were higher in group A than those in group B(P<0.01).When the cut-off value of PCT was 0.64 ng/mL,the sensitivity, specificity and AUC for the diagnosis of children with sepsis were 82.5%,72.5% and 0.813 [95%CI(0.744-0.881)],respectively.When the cut-off value of combined diagnostic model was 0.401,the sensitivity, specificity and AUC for the diagnosis of children with sepsis were 90.0%,85.0% and 0.908 [95%CI(0.858-0.958)],respectively.ConclusionCombined detection of NLR,PLR and RDW has higher diagnostic value for the children with sepsis.
