摘要
目的分析川崎病患儿冠状动脉扩张(CAE)诊断中,心率变异性(HRV)、中性粒细胞与淋巴细胞比值(NLR)的联合预测价值。方法选取146例川崎病患儿,根据影像学检测结果分为无CAE组(50例)和有CAE组(96例),对所有患儿进行超声心动图检测和血液检测,分析HRV与NLR变化。结果两组HRV时域性与频域性指标相比,CAE组患儿水平均显著低于无CAE组(P<0.05或<0.01)。随着CAE程度的加重,患儿的时域指标水平均下降,频域指标中VLF、LF、HF水平下降,LF/HF水平上升(P<0.05);HRV+NLR诊断的敏感度、特异度、准确率均高于单独检测(P<0.01)。结论临床可采用HRV和NLR联合检测方式,诊断川崎病患儿CAE的发生情况,HRV时域指标、频域指标下降,NLR水平上升提示川崎病患儿发生CAE。
Objective To analyze the combined predictive value of heart rate variability(HRV)and neutrophil to lymphocyte ratio(NLR)in the diagnosis of coronary artery dilatation(CAE)in children with Kawasaki disease.Methods A total of 146 children with Kawasaki disease admitted to our hospital were selected and divided into two groups according to the imaging results:no CAE group(50 cases)and CAE group(96 cases).Echocardiography and blood tests were performed on all children,and the changes of HRV and NLR were analyzed.Results Compared with the time and frequency of HRV in the two groups,the levels of the CAE group were significantly lower than those in the non-CAE group(P<0.05 or<0.01).With the aggravation of CAE degree,the time domain indexes of children decreased,the frequency domain indexes of VLF,LF,HF decreased,and LF/HF increased(P<0.05).The sensitivity,specificity,and accuracy of HRV+NLR diagnosis were all higher than the detection alone(P<0.01).Conclusion The combined detection of HRV and NLR can be used to predict the occurrence of CAE in children with Kawasaki disease.The decrease of HRV time domain index and frequency domain index,and the increase of NLR level indicate the occurrence of CAE in children with Kawasaki disease.
