摘要
目的对比不同剂量注射用牛肺表面活性剂(CPS)联合双水平无创正压(BiPAP)通气治疗新生儿呼吸窘迫综合征(NRDS)的效果。方法回顾性分析唐河县人民医院2017年1月至2020年12月收治的85例NRDS患儿资料,按给药剂量分成A组(43例)和B组(42例)。两组均行BiPAP通气治疗,于此基础上B组接受常规剂量注射用CPS治疗,A组接受大剂量注射用CPS治疗。对比两组总有效率、治疗前及治疗3 d血气指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、动脉/肺泡氧分压(a/APO_(2))]、炎症因子[白细胞介素-8(IL-8)、白细胞介素-10(IL-10)]水平和不良反应(一过性发绀、血压波动、心率波动、呛咳等)发生率。结果A组总有效率[95.35%(41/43)]较B组[78.57%(33/42)]高(P<0.05);治疗3 d,两组PaO_(2)、a/APO_(2)较治疗前升高,且A组PaO_(2)、a/APO_(2)高于B组,PaCO_(2)较治疗前降低,且A组PaCO_(2)低于B组(P<0.05);治疗3 d,两组血清IL-8水平较治疗前降低,且A组低于B组,血清IL-10水平较治疗前升高,且A组高于B组(P<0.05);两组不良反应发生率对比,差异无统计学意义(P>0.05)。结论与常规剂量注射用CPS联合BiPAP通气治疗NRDS患儿相比,大剂量注射用CPS治疗效果更为显著,于改善血气水平、缓解炎症反应方面更具优势,且未明显增加不良反应。
Objective To compare the effects of different doses of calf pulmonary surfactant(CPS)for injection combined with bilevel positive airway pressure(BiPAP)ventilation in the treatment of neonatal respiratory distress syndrome(NRDS).Methods A total of 85 children with NRDS in Tanghe People’s Hospital from January 2017 to December 2020 were retrospectively collected and divided into group A(43 cases)and group B(42 cases).The two groups were treated with BiPAP ventilation.Group B was treated with conventional CPS for injection,and group A was treated with large dose of CPS.The total effective rate,blood gas indexes[arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxde(PaCO_(2)),arterial to alveolar oxgen partial pressure ratio(a/APO_(2))],inflammatory factors[interleukin-8(IL-8),interleukin-10(IL-10)]and incidence of adverse reactions(transient cyanosis,blood pressure fluctuation,heart rate fluctuation,cough,etc)were compared between the two groups.Results The total effective rate[95.35%(41/43)]of group A was higher than that[78.57%(33/42)]of group B(P<0.05).After 3 days of treatment,PaO_(2)and a/APO_(2)in the two groups were higher than those before treatment,and PaO_(2)and a/APO_(2)in group A were higher than those in group B(P<0.05).PaCO_(2)in the two groups were lower than those before treatment,and PaCO_(2)in group A was lower than that in group B(P<0.05).After 3 days of treatment,the serum IL-8 levels in the two groups were lower than those before treatment,and the serum IL-8 level in group A was lower than that in group B(P<0.05).The serum IL-10 levels in the two groups were higher than those before treatment,and the serum IL-10 level in group A was higher than that in group B(P<0.05).There was no statistical difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Compared with the conventional dose of CPS combined with BiPAP ventilation in the treatment of children with NRDS,the application of large dose of CPS in the treatment of NRDS is mo
