摘要
目的探讨牛肺表面活性剂联合氨溴索治疗新生儿呼吸窘迫综合征(NRDS)的效果。方法抽取2021年6月至2022年6月商丘市第四人民医院收治的NRDS患儿30例,按照随机数字表法分为单一组和联合组,每组15例。单一组采用牛肺表面活性剂治疗,联合组在单一组基础上使用氨溴索治疗。比较两组患者治疗痊愈比例,比较两组治疗前后血气指标[二氧化碳分压(PaCO_(2))、氧分压(PaO_(2))、吸入氧浓度(FiO_(2))]水平,比较两组上机时间、氧疗时间、住院时间及并发症发生情况。结果联合组治疗痊愈比例(14/15)高于单一组(8/15),P<0.05。治疗后,联合组PaCO_(2)、FiO_(2)水平低于单一组,PaO_(2)水平高于单一组(P<0.05)。联合组上机时间、氧疗时间、住院时间均少于单一组(P均<0.05)。联合组气胸、颅内出血、肺出血发生比例(1/15,0/15,0/15)与单一组(3/15,1/15,2/15)比较差异未见统计学意义(P>0.05)。结论牛肺表面活性剂联合氨溴索治疗NRDS效果显著,可明显改善患儿血气指标,缩短上机和氧疗时间,且并发症少,安全性良好。
Objective To investigate the effect of calf pulmonary surfactant combined with ambroxol in the treatment of neonatal respiratory distress syndrome(NRDS).Methods A total of 30 children with NRDS treated in the Fourth People’s Hospital of Shangqiu from June 2021 to June 2022 were selected,and they were divided into single group and combined group according to the random number table method,with 15 cases in each group.The cure ratio of the two groups were compared;the blood gas indexes,including arterial partial pressure of carbon dioxide(PaCO_(2)),arterial partial pressure of oxygen(PaO_(2))and fraction of inspired oxygen(FiO_(2)),before and after treatment of the two groups were compared;the time of device,oxygen therapy time,hospital stay and the complications occurrence of the two groups were compared.Results The cure ratio of the combined group was 14/15,which was higher than the 8/15 of the single group(P<0.05).After treatment,the levels of PaCO_(2)and FiO_(2)in the combined group were lower than those in the single group,and the level of PaO_(2)was higher than that in the single group(P<0.05).Compared with the single group,the time of device,oxygen therapy and hospital stay in the combined group were shorter than those in the single group(all P<0.05).There was no significant difference in the ratio of pneumothorax,intracranial hemorrhage and pulmonary hemorrhage between the combined group(1/15,0/15,0/15)and the single group(3/15,1/15,2/15),P>0.05.Conclusions Calf pulmonary surfactant combined with ambroxol in the treatment of NRDS has a significant effect,it can significantly improve the blood gas indexes of children,shorten the time of device and oxygen therapy,and has few complications and good safety.
