摘要
目的研究呼吸窘迫综合征(respiratory distress syndrome,RDS)早产儿联合采用布地奈德、肺表面活性物质(PS)治疗对支气管肺发育不良(BPD)的预防作用。方法对2020年1月至2023年1月河南省人民医院新生儿科接收的34例RDS早产儿的临床资料予以回顾性分析,按照治疗方法进行分组,将予PS治疗的16例患儿设为对照组,而予PS结合布地奈德治疗的18例患儿设为联合组。比较两组血气分析指标、BPD发生率、吸氧与呼吸机使用情况及并发症。结果治疗后,两组血气分析指标对比,差异性显著(P<0.05);与对照组比较,联合组BPD发生率明显降低,有明显差异性(P<0.05);两组吸氧、呼吸机用时比较存在统计学价值(P<0.05);联合组并发症发生率少于对照组,但无显著差异性(P>0.05)。结论对RDS早产儿采用布地奈德辅以PS治疗能有效改善血气指标、预防BPD,还能抑制局部炎症,使吸氧及呼吸机用时缩短,值得临床推广。
Objective To study the preventive effect of combined treatment of budesonide and pulmonary surfactant(PS)on bronchopulmonary dysplasia(BPD)in premature infants with respiratory distress syndrome(RDS).Methods The clinical data of 34 premature infants with RDS admitted to the neonatology department of our hospital from January 2020 to January 2023 were retrospectively analyzed and were divided into two groups according to treatment methods.16 children treated with PS were set as the control group,while 18 children treated with PS combined with budesonide were set as the combination group.Blood gas analysis indexes,incidence of BPD,oxygen intake and ventilator use,and complications were compared between the two groups.Results After treatment,blood gas analysis indexes of the two groups were compared,and the difference was significant(P<0.05).Compared with the control group,the incidence of BPD in the combination group was lower,with significant difference(P<0.05).The time of oxygen inhalation and ventilator between the two groups was statistically significant(P<0.05).Complications in combination group were less than those of control group,and there was no significant difference(P>0.05).Conclusion The treatment of premature infants with RDS with budesonide combined with PS can effectively improve blood gas indexes,prevent BPD,inhibit local inflammation and shorten the time of oxygen inhalation and ventilator,which is worthy of clinical promotion.
