摘要
目的系统评价香菇多糖用于肿瘤化疗患者的疗效及安全性,为其临床应用提供循证参考。方法系统检索“Cochrane Library、Pubmed、Embase、Web of Science、CNKI、VIP、万方、CBM”数据库,用RevMan 5.3软件进行meta分析,对近期有效率、Karnofsky生活质量(KPS)评分改善情况、T细胞亚群CD3细胞、CD4细胞、CD8细胞、CD4/CD8,NK细胞活性及7种不良反应进行效应合并。结果共纳入21篇文献,共计1633例患者。Meta分析结果显示:香菇多糖对肿瘤患者可以提高化疗近期有效率(RR=1.31,95%CI 1.18~1.45,P<0.001)、提高KPS评分(RR=1.71,95%CI 1.54~1.90,P<0.001),能增强成熟T淋巴细胞(CD3,P<0.001)、辅助性T细胞(CD4,P=0.004)活性,提高CD4/CD8(P<0.001)比值,并对NK细胞有强化作用(P=0.001),联合香菇多糖可以减少化疗引起的毒性反应。结论香菇多糖在联合常规化疗方案,可以提高临床近期有效率、改善生活质量、增强免疫能力,但加大香菇多糖剂量对减少骨髓抑制作用不大,应严格控制用法用量,注意合理用药。
OBJECTIVE To systematically evaluate the efficacy and safety of lentinan in tumor chemotherapy patients,and to provide evidence for clinical reference.METHODS The Cochrane Library,Pubmed,Embase,Web of Science,CNKI,VIP,Wanfang and CBM databases were comprehensively searched,and the meta-analysis was performed with RevMan 5.3 software.The immediate effec tive rate,the improvement of Karnofsky Performance Status(KPS)Score,T cells ubsets of CD3 cells,CD4 cells and CD8 cells,CD4/CD8,the activity of NK cells and the effects of 7 adverse reactions were combined to evaluate.RESULTS A total of 44 articles were collected,including 3519 patients.The meta-analysis illustrated that lentinan can enhance the immediate effective rate(RR=1.31,95%CI 1.18~1.45,P<0.001),the KPS score(RR=1.71,95%CI 1.54~1.90,P<0.001),the activity of mature T lymphocytes(CD3,P<0.001)and helper T cells(CD4,P=0.004),enhancing the ratio of CD4/CD8(P<0.001),an d strengthening the function of NK cells(P=0.001)in patients with tumor chemotherapy.Combined application of lentinan can reduce the adverse reactions.CONCLUSIONS Lentinan in combination with conventional chemotherapy regimen can improve clinical short-term efficacy,the quality of life and the immune ability.However,increasing the dose of letinous edodes polysaccharide is not very effective in reducing bone marrow suppression.So the usage and dosage should be strictly controlled and pay attention to rational use of drugs.
