摘要
目的了解资中县新发现艾滋病病毒(HIV)感染者首次CD4+T淋巴细胞(简称CD4细胞)数量,为制定艾滋病防控策略提供依据。方法利用流式细胞仪测定2016-2018年资中县新发现的HIV感染者首次CD4细胞检测658例的结果,分析新发现HIV感染者的免疫状况和艾滋病的临床分期。结果658例检测者CD4、CD8、CD3细胞中位数分别为204.0个/μL、678.0个/μL、979.5个/μL;重度免疫缺陷320例(48.63%),中度免疫缺陷195例(29.64%),轻度免疫缺陷88例(13.37%),无免疫缺陷55例(8.36%);HIV感染早期55例(8.36%),感染中期283例(43.01%),艾滋病期320例(48.63%)。2016-2018年不同年份CD4、CD8、CD3细胞中位数差异有统计学意义(χ^2=31.376,P<0.05;χ^2=28.542,P<0.05和χ^2=36.310,P<0.05)。<30岁、30~59岁、≥60岁三个年龄段CD4、CD8、CD3细胞中位数差异有统计学意义(χ^2=19.037,P<0.05;χ^2=37.497,P<0.05和χ^2=16.717,P<0.05)。结论资中县HIV感染者中度和重度免疫缺陷比例较高,发现较晚。CD4、CD8、CD3细胞中位数2016年比2017年和2018年高。<30岁年龄组CD4细胞中位数最高。
Objective To know the status of CD4+T lymphocytes(CD4 cells)tested for the first time of the HIV infection in Zizhong county,and to provide evidence for the development of AIDS prevention and control strategies.Methods The flow cytometry was used to test the CD4 cells of 658 cases of newly detected HIV infected for the first time during 2016-2018.The immune status and clinical stages were analyzed accordingly.Results The median of CD4,CD8 and CD3 were 204.0 cells/μL,678.0 cells/μL and 979.5 cells/μL respectively in the 658 patients,with 320(48.63%)in severe immune deficiency,195(29.64%)in moderate immune deficiency,88(13.37%)in mild immune deficiency,55(8.36%)in non-immune deficiency,55(8.36%)in early stage of HIV infection,283(43.01%)in middle stage and 320(48.63%)in AIDS stage.The differences in the median of CD4,CD8 and CD3 in different years from 2016 to 2018 were statistically significant(χ^2=31.376,P<0.05;χ^2=28.542,P<0.05 andχ^2=36.310,P<0.05).The median CD4,CD8 and CD3 of the three age groups<30 years old,30-59 years old and≥60 years old were significantly different(χ^2=19.037,P<0.05;χ^2=37.497,P<0.05 andχ^2=16.717,P<0.05).Conclusion The proportion of HIV infected people with moderate and severe immunodeficiency is relatively high,and the detection is late in Zizhong.The median of CD4,CD8 and CD3 in 2016 is higher than those in 2017 and 2018.The age group<30 years have the highest median of CD4.
