香菇多糖联合奈达铂和多西他赛对非小细胞肺癌合并肺结核患者血清炎性因子水平的影响 被引量:18

Effect of lentinan combined with nedaplatin on serum inflammatory factors in patients with nonsmall cell lung cancer complicated with pulmonary tuberculosis

摘要 目的探讨香菇多糖联合奈达铂和多西他赛对非小细胞肺癌合并肺结核患者血清炎性因子水平的影响。方法选取2016年4月至2018年1月间上海复旦大学附属中山医院青浦分院收治的106例非小细胞肺癌(NSCLC)合并肺结核患者,采用随机数表法分为化疗组和联合组,每组53例。两组患者均接受6个月的抗结核治疗和多西他赛+奈达铂(DN)方案化疗。联合组患者在化疗的同时采用香菇多糖注射液治疗。观察比较两组患者治疗后的疗效评价、不良反应发生情况和血清炎性因子水平。结果两组患者治疗后的疗效进行评价,化疗组临床稳定率为75.5%,小于联合组的90.6%,差异有统计学意义(P<0.05);化疗组治疗有效率为26.4%,小于联合组的52.8%,差异有统计学意义(P<0.05)。联合组腹泻和恶心呕吐发生率分别为58.5%和75.5%,显著低于化疗组的81.1%和90.6%,差异均有统计学意义(均P<0.05)。两组患者治疗后白细胞介素-4(IL-4)和高敏C反应蛋白(hs-CRP)均显著降低,IL-2和干扰素-γ(IFN-γ)均显著升高,差异均有统计学意义(均P<0.05);联合组患者治疗后IL-4和hs-CRP均显著低于化疗组,IL-2和IFN-γ均显著高于化疗组,差异均有统计学意义(均P<0.05)。结论香菇多糖联合奈达铂和多西他赛能有效调节患者的机体免疫力,改善患者的抗病毒能力,降低不良反应发生率,提高治疗效果,值得临床推广及应用。 Objective To examine the effect of lentinan combined with nedaplatin on serum inflammatory factors in patients with non-small cell lung cancer(NSCLC)complicated with pulmonary tuberculosis(TB).Methods A total of 106 patients with NSCLC combined with TB were enrolled at Fudan University Affiliated Zhongshan Hospital Qingpu Branch from April 2016 to January 2018.The patients were divided into a chemotherapy group and a combination group according to the random number table method with 53 patients in each group.Both groups received anti-tuberculosis treatment and chemotherapy regimen with docetaxel plus nedaplatin(DN)for 6 months.Apart from this,the combination group was intravenously infused with lentinan injection during the chemotherapy.The efficacy evaluation,adverse reactions and serum inflammatory factors were compared between the two groups before and after treatment.Results The clinical stability rate was 75.5%in the chemotherapy group which was significantly lower than 90.6%of the combination group(P<0.05).The efficacy rate was 26.4%in the chemotherapy group,which was significantly lower than 52.8%of the combination group(P<0.05).The incidence of diarrhea and nausea and vomiting was 58.5%and 75.5%,respectively in the combination group,which was significantly lower than 81.1%,90.6%of the chemotherapy group,respectively(all P<0.05).After the treatment,the levels of interleukin-4(IL-4),and high-sensitivity C-reactive protein(hs-CRP)decreased in both groups and IL-2 and interferon-γ(IFN-γ)increased in both groups(all P<0.05).IL-4 and hs-CRP were lower in the combination group than in the chemotherapy group and IL-2 and IFN-γwere higher in the combination group than in the chemotherapy group(all P<0.05).Conclusion Lentinan combined with nedaplatin can effectively regulate the immunity system,improve their antiviral ability,reduce the incidence of adverse reactions and improve the therapeutic efficacy.It is worthy of clinical promotion and application.
