摘要
目的了解山西省基层卫生人员对县乡医疗卫生机构一体化改革的知晓率,了解一体化改革在推进过程中的问题及对策,为政府决策提供依据。方法随机抽取某省县、乡、村三级医疗卫生机构专业人员1464名,进行问卷调查;并对各县医疗集团院长、医保局局长、乡镇卫生院院长、村医等进行个体访谈。结果70.4%人员听说过县乡医疗机构一体化改革,部分措施如“实行医保总额打包付费”等问题的知晓率不足50%。结论县乡医疗卫生机构一体化改革取得了一定效果,但是基层卫生人员对改革的知晓率有待提高;可通过加大省市级政府的投入、真正落实县医疗集团用人自主权、进一步理清县卫体局与县医疗集团的职责、加强省市级医院对县医疗集团的帮扶作用、相关部门加大对一体化改革的宣传等措施,推进这项改革的进程。
Objective To determine the awareness rate of primary medical personnel on the integrated reform of rural medical and health institutions in Shanxi province,and to investigate problems and solutions in the process of integrated reform,so as to provide evidence for government decision-making.Methods A total of 1464 primary medical professionals from medical and health institutions at county,township and village levels in a certain province were randomly included in the study,and a questionnaire survey was conducted.Individual interviews were conducted with directors of the county medical institutions,medical insurance bureaus,township health bureaus and village doctors.Results 70.4%of the professionals had heard of the integrated reform of rural medical and health institutions.The awareness rate of some measures such as"medical insurance package payment"was less than 50%.Conclusion The integrated reform of rural medical and health institutions has achieved certain results,but the awareness rate of primary medical personnel on the reform needs to be improved.The advancement of this reform process may be achieved by increasing the investment of provincial and municipal governments,truly implementing the policy of granting decision-making power to county medical institutions for the employment of working personnel,further clarifying the responsibilities of county health and sports bureaus and county medical institutions,strengthening the assistance of provincial and municipal hospitals to county medical institutions,and enhancing the publicity of the integrated reform through relevant departments.
