摘要
目的:比较甲状腺腺叶切除术与甲状腺次全切术治疗甲状腺结节患者的效果。方法:回顾性分析2017年8月至2019年1月100例甲状腺结节患者的临床资料,按照手术方案不同分为对照组与研究组各50例,对照组给予甲状腺次全切术治疗,研究组给予甲状腺腺叶切除术治疗,比较两组手术相关指标水平、并发症发生率、血清学指标(甲状腺过氧化物酶、血清促甲状腺激素)水平和术后半年复发率。结果:研究组手术时间和住院时间短于对照组,术中出血量少于对照组,差异均有统计学意义(P<0.05);研究组并发症发生率为6.0%,明显低于对照组的20.0%,差异有统计学意义(P<0.05);术后,研究组甲状腺过氧化物酶水平低于对照组,血清促甲状腺激素水平高于对照组,差异均有统计学意义(P<0.05);随访半年,两组均无复发。结论:甲状腺腺叶切除术治疗甲状腺结节患者可缩短手术时间和住院时间,减少术中出血量,降低并发症发生率,以及改善血清学指标水平,效果优于甲状腺次全切术治疗效果。
Objective:To compare effects of thyroid lobectomy and subtotal thyroidectomy in treatment of patients with thyroid nodules.Methods:A retrospective analysis of the clinical data of 100 patients with thyroid nodules from August 2017 to January 2019 was performed.According to different surgical plans,they were divided into control group and study group,each with 50 cases.The control group was treated with subtotal thyroidectomy,while the study group was treated with thyroid lobectomy.The levels of surgery-related indicators,the incidence of complications,the levels of serological indicators(thyroid peroxidase,serum thyroid stimulating hormone)and the recurrence rate were compared between the two groups.Results:The operation time and hospital stay in the study group were shorter than those in the control group;the intraoperative blood loss was less than that in the control group;and the differences were statistically significant(P<0.05).The complication rate in the study group was 6.0%,which was significantly lower than 20.0%in the control group,and the difference was statistically significant(P<0.05).After the surgery,the level of thyroid peroxidase in the study group was lower than that in the control group;the level of serum thyroid stimulating hormone was higher than that in the control group;and the differences were statistically significant(P<0.05).Followed up for half a year,there was no recurrence in both groups.Conclusions:Thyroid lobectomy in the treatment of thyroid nodules can shorten the operation time and hospital stay,reduce the intraoperative blood loss,reduce the incidence of complications,and improve the levels of serological indicators.Moreover,it is superior to subtotal thyroidectomy.
