摘要
目的探索医联体背景下丽水市老年人家庭医生服务续签意愿及影响因素。方法 2019年6-8月,采用随机抽样法对丽水市5个社区卫生服务中心和10处老年人密集场所签约老年人进行问卷调查。问卷内容包括基本情况、认知情况、利用情况、续签意愿及原因等。共发放问卷650份,回收有效问卷590份,有效回收率90.8%。结果 82.5%的老年人愿意续签。logistic回归分析显示,是否有慢性病、认为家庭医生是否必要、是否是家庭医生接待就诊、过去一年家庭医生主动联系次数以及首诊地点为老年人续签意愿影响因素,差异有统计学意义(P<0.05)。结论丽水市签约老年人续签意愿较高。认为家庭医生是否必要、是否具有慢性病以及家庭医生服务利用情况是影响老年人续签的主要因素。患有慢性病、认为家庭医生有必要、家庭医生接待就诊、过去一年家庭医生主动联系5次以上和首诊地点为基层医疗机构的老年人更倾向于续签。
OBJECTIVE To contracted residents’ intention of renewing the contract with family doctors and associated factors in Lishui City. METHODS From June 2019 to August 2019,5 community health service centers and 10 elderly-intensive places were selected in Lishui City by random sampling. The content of questionnaire included sociodemographic characteristics,awareness,utilization,renewal intention and its reasons. The survey obtained a response rate of 90.8%. RESULTS The prevalence of intending to renew the contract was 82.5%. Logistic regression analysis demonstrated that,chronic disease,attitude to family doctor service system’s necessity,family doctor treat,active contacts times of family doctors in past year,choose the contracted family doctor as a first contact had great impact on contracted residents’ intention to renew the contract(P<0.05). CONCLUSION The intention of renewing the contract with family doctors in Lishui was high. The thought of importance,chronic disease,family doctor service’s utilization were the main factors.Young Residewts with chronic diseases,who considered family doctors as the necessarity,nsifed family doctors,contacted by the family doctors more than 5 times once a year,on taking primary institutions as the first dignosis place,preferred to renew the contrat.
