摘要
[目的]比较冷冻与脱钙冻干异体骨移植物修复兔桡骨缺损的成骨能力和生物力学特性。[方法]制备冷冻和脱钙冻干异体骨移植物,分别植入各16只新西兰白兔的双侧桡骨中段1.5cm缺损处。于术后2、4、8、12周处死动物,行大体观察、X线及HE染色组织学观察评分,形态计量新生骨量;取静脉血检测BGP;术后12周对移植骨段进行生物力学测试。[结果]移植骨大体形态学观察可见各组移植骨位置良好,术后12周均可与宿主骨形成牢固愈合。术后4周和8周,脱钙冻干组的X线评分、组织学评分及成骨计量均显著优于冷冻组(P<0.05),但术后12周两组无显著差别(P>0.05)。术后4周和8周,脱钙冻干组血浆BGP水平显著高于冷冻组(P<0.05);术后12周两组间差异已无统计学意义(P>0.05)。术后12周生物力学测试,脱钙冻干组在三点弯曲试验的最大弯曲力和抗压强度,以及扭转试验的最大扭矩和抗扭强度均显著不及冷冻组(P<0.05)。[结论]与冷冻骨相比,脱钙冻干异体骨具有优良的成骨活性,但力学强度下降。
[Objective]To compare the osteogenic capacity and biomechanical characteristics of fresh frozen bone allograft versus decalcified freeze-dried bone allograft for repairing radius segment bone defect in rabbit.[Methods]Fresh frozen bone allografts and decalcified freeze-dried bone allografts 1.5 cm in length were prepared,and then were implanted into the radius defects 1.5 cm in length in 32 New Zealand rabbits,16 animals for each graft.At 2,4,8 and 12 weeks after operation,the animals were sacrificed respectively and the radius samples retrieved for gross observation,X-ray and histological examinations with HE staining to calculate the X-ray and histological scores and morphometric assay of new bone formation.Vein blood was taken for measurement of plasm bone Gla protein(BGP).In addition,biomechanical tests were conducted on the samples harvested at 12 weeks after operation.[Results]The gross observation revealed that all the bone grafts remained in proper position and got solid healing with the host bone at 12 weeks in both groups.The freeze-dried group got significantly higher X-ray and histological scores,as well as greater new bone formation than the frozen group at 4 and 8 weeks(P<0.05),which became not statistically significant at 12 weeks(P>0.05).The freeze-dried group had significantly higher level of plasm BGP than the frozen group at 4 and 8 weeks(P<0.05),which also turned to be not statistically significant at 12 weeks(P>0.05).In term of biomechanical assay,the freeze-dried group proved significantly inferior to the fresh frozen group in strengths in three-point bending test and torsion test(P<0.05).[Conclusion]The decalcified freeze-dried bone allograft has superior osteogenous active,whereas inferior mechanical strength to the fresh frozen bone allograft.
