摘要 目的探究4周肩胛带及上肢D2模式本体感觉神经肉促进技术(proprioceptive neuromuscular facilitation,PNF)干预对肩胛骨静态、特殊角度位置及上肢功能的影响。方法选取北京体育大学患有肩胛骨动力障碍(scapular dyskinesis,SD)的学科女生16人,随机分为实验组(T组)和对照组(C组)。T组进行4周上肢PNF干预,C组不做干预,2组受试者实验期间均保持正常的活动。干预前后对肩胛骨平衡角(scapula balance angle,SBA),肩胛侧方滑移测试(lateral scapular slide test,LSST),肩关节功能指标(DASH评分,Y-Balance)测试,进行组内和组间的比较。结果与干预前相比(1)T组SBA明显减小(P=0.01),C组前后无显著性差异;(2)LSST中,T组外展90°位肩胛下角向外滑移减少(P=0.028),C组无显著性差异;(3)肩关节功能指标:①T组和C组干预前后DASH评分均无差异。②T组Y-Balance成绩分数显著提高(P=0.002),且在3个方向:内侧线(P=0.005)、外上线(P=0.008)、外下线(P=0.003)测试距离均有增加。结论4周PNF干预可有效改善肩胛骨静态和动态位置:表现为肩胛骨平衡角减小,肩外展90°时肩胛骨外移减少;肩胛骨位置的改善能有效提高上肢功能。 Objective To investigate the rehabilitation effect on scapular dyskinesis with PNF.Methods Sixteeen female students with SD randomly assigned to two groups:test group and control group.Four weeks PNF treatment was used to test group,while nothing for control group.Students in two groups kept normal activities during the test.Scapular balance angle,lateral scapular slide test and functional index of shoulder(DASH questionnaire and Y-Balance test)were tested before and after the study.Then analysis the difference between before and after in the same group and the two groups.Results Compared with the results before the test(1)the balance angle in test group decreased and the control group didn't.(2)During LSST,when hands put beside the body and shoulder abduction to 90 degree,scapular external slide decreased in test group,while no significant difference in control group.(3)Functional index of shoulder:①there was no significant difference in DASH in two groups;②Y-Balance test improved in three directions in test group.Conclusions Four weeks'PNF intervention can effectively improve the scapula static and dynamic position,performance as the scapular balance anglereduction,and scapula slide reducewhen static and shoulder abduction 90 degree;improvement of scapula position is helpful to improve upper limb function.
