摘要
目的探讨MMP-9、VEGF在膝关节骨性关节炎软骨中的表达及意义。方法选取2018年10月—2020年1月宁夏医科大学总医院膝关节骨关节炎患者行人工全膝关节置换术35例,作为观察组;膝关节外伤患者行关节镜手术治疗13例,作为对照组。观察组取术中关节成形截除的股骨髁软骨,对照组取因损伤脱落需要清理的股骨髁软骨。采用苏木素-伊红染色、番红O-固绿特殊染色在光镜下观察软骨病理改变及免疫组化检测MMP-9、VEGF表达的变化。结果苏木素-伊红染色结果,观察组软骨表面不光整,有缺损,软骨细胞形态大小不等,潮线不完整;对照组软骨表面光整,软骨层排列均匀,潮线较连续。番红O-固绿染色结果显示,观察组软骨胶原显色较弱,软骨细胞排列不均匀,软骨细胞大小不等,成团状,潮线不连续;对照组软骨胶原显色较强,软骨细胞大小较均匀,排列较规整,潮线连续。免疫组化结果显示:免疫组化染色阳性细胞主要在软骨细胞胞浆和基质周围,在退变软骨组织中观察组的MMP-9阳性表达量为0.74±0.08,高于对照组(0.33±0.07)(P<0.01);观察组的VEGF阳性表达量为0.59±0.03,高于对照组(0.39±0.05)(P<0.05)。结论膝关节骨性关节炎软骨中MMP-9、VEGF阳性表达明显增高,表明其与软骨退行性病变有关。
Objective To explore the expression of MMP-9 and VEGF in the cartilage of knee osteoarthritis.Methods From October 2018 to January 2020,35 patients were selected from General Hospital of Ningxia Medical University as the observation group,who were treated with total knee arthroplasty due to knee osteoarthritis,and 13 patients were selected as the control group who were treated with arthroscopic surgery due to knee trauma.In the observation group,the femoral condylar cartilage was removed by arthroplasty,while in the control group,the femoral condylar cartilage was removed due to injury.The pathological changes of cartilage were observed by hematoxylin eosin staining and saffron o-green staining under light microscope,and the expression of MMP-9 and VEGF were detected by immunohistochemistry.Results It was showed that the cartilage surface of the observation group was not smooth,there were defects,the shape and size of chondrocytes were different,and the tide line was not complete;the cartilage surface of the control group was smooth,the cartilage layer was evenly arranged,and the tide line was relatively continuous by HE staining.In the observation group,collagen color were weak,chondrocytes were not evenly distributed,layered,clustered,and tidal line was not continuous;in the control group,collagen color were strong,chondrocytes were of uniform size,regular arrangement,and tidal line continued by saffron o-green staining.The results of immunohistochemistry showed that the positive cells of immunohistochemical staining were mainly around the cytoplasm and matrix of chondrocytes.The MMP-9 positive expression in the degenerated cartilage tissue in the observation group was 0.74±0.08,higher than that in the control group(0.33±0.07)(P<0.01);the positive expression of VEGF in degenerated cartilage tissue in the observation group was 0.59±0.03,higher than that in the control group(0.39±0.05)(P<0.05).Conclusion It is significant that the positive expression of MMP-9 and VEGF in cartilage of knee osteoarthritis is in
