基质金属蛋白酶MMP-2、MMP-9与类风湿性关节炎关节破坏的关系 被引量:18

Association of serum MMP-2 and MMP-9 and progression of joint damage in early rheumatoid arthritis

摘要 目的:观察血清基质金属蛋白酶MMP-2、MMP-9与类风湿性关节炎患者(Rheumatoid arthritis,RA)关节破坏的关系。方法:用双抗体夹心ELISA法测定90例未经治疗的早期RA患者及20例健康人血清中MMP-2、MMP-9,并分析其与RA患者X线表现积分(ΔLarsen)及健康评分(ΔHAQ)、CRP的关系。结果:血清MMP-2、MMP-9水平与C反应蛋白(CRP)、ΔLarsen、ΔHAQ显著相关,其相关系数及P值分别为r=0·40和0·46,P<0·01;r=0·28和0·31,P<0·05;r=0·30和0·34,P<0·01。18例最初CRP正常的患者,MMP-2及MMP-9水平与观察12个月后出现关节侵蚀改变相关(r=0·59和0·48,P<0·05)。回归分析发现,最初无骨侵蚀改变的患者,Larsen积分显示的骨侵蚀进展与MMP-2含量密切相关(r=0·32,P<0·01)。结论:血清MMP-2、MMP-9水平与RA患者的病情活动及早期关节的骨侵蚀显著相关,可以作为无骨破坏的早期RA患者出现骨侵蚀的有益预测指标。 Objective:To investigate the correlation of serum MMP-2 and MMP-9 with the progression of joint damage in early rheumatoid arthritis. Methods:Serum MMP-2 and MMP-9 in ninety patients with early RA and 20 normal controls were tested with a douhle-antibedy enzyme-linked immunosorhent assay. The correlation between serum MMP-2 and MMP-9 and Larsen score(△Lamen) and Health Assessment Questionnaire(△HAQ) over the first 12 monthswas analysed. Results:Serum levels of MMP-2 and MMP-9 correlated significantly with C-reactive protein (CRP) (r = 0. 40 and 0. 46, P 〈 0. 01 ), △HAQ (r = 0. 28 and 0. 31,P 〈 0. 01 ) and △Larsen (r =0. 30 and 0. 34, P 〈 0. 05) respectively. Analysis of the group of patients with a normal CRP showed correlation of MMP-2 and MMP-9 with the presence of erosive disease( r = 0. 59 and 0. 48 respectively,P 〈 0. 05). Logistic regression analysis in the patients who were non-erosive at presentation,showed that the strongest correlation with progression in Larsen score was the MMP-2 level( r = 0. 32, P 〈0. 01). Conclusion: Serum MMP-2 and MMP-9 levels correlate with disease activity and early progression of joint damage. They may be a valuble predictor of the progression of erosive disease in patients who are not erosive at presentation.
