MRI联合MMP-9、IL-7R对膝关节骨性关节炎发生发展的诊断价值研究

摘要 目的分析磁共振成像(magnetic resonance imaging,MRI)联合基质金属蛋白酶-9(matrix metallo proteinase-9,MMP-9)、白介素7受体(Interleukin 7 receptor,IL-7R)对膝关节骨性关节炎发生发展的诊断价值。方法选取2020年1月—2021年1月重庆市沙坝区人民医院收治的膝关节骨性关节炎患者60例为研究组,另选体检者40例为对照组。两组均行MRI检查及MMP-9、IL-7R水平检测。结果研究组ADC值、MMP-9、IL-7R水平高于对照组(P<0.05)。与研究组轻度患者相比,中度、重度患者ADC值、MMP-9、IL-7R水平逐渐升高(P<0.05)。预后不良患者ADC值、MMP-9、IL-7R水平高于预后良好患者(P<0.05)。相比于ADC值、MMP-9、IL-7R的单项诊断,联合诊断对膝关节骨性关节炎诊断及预后不良预测的敏感度、准确性均较高(P<0.05)。结论MRI联合MMP-9、IL-7R对膝关节骨性关节炎诊断价值及预后不良预测价值较高,可为膝关节骨性关节炎诊治提供借鉴。 Objective To analyze the diagnostic value of magnetic resonance imaging(MRI)combined with matrix metalloproteinase-9(MMP-9)and interleukin 7 receptor(IL-7R)in the development of knee osteoarthritis.Methods From January 2020 to January 2021,60 patients with knee osteoarthritis who were treated in our hospital were selected as the research group,and 40 volunteers who underwent physical examination in our hospital were selected as the control group.Results MRI and MMP-9 and IL-7R levels were detected in both groups.Compared with the control group,the ADC value,MMP-9 and IL-7R levels in the study group were higher(P<0.05).Compared with mild patients in the study group,the ADC value,MMP-9 and IL-7R levels in moderate and severe patients were higher(P<0.05);The ADC,MMP-9 and IL-7R levels in severe patients were higher than those in moderate patients(P<0.05).Compared with patients with good prognosis in the study group,patients with poor prognosis had higher ADC value,MMP-9 and IL-7R levels(P<0.05).Compared with ADC value,MMP-9 and IL-7R single diagnosis,combined diagnosis has higher sensitivity,specificity and accuracy in the diagnosis of knee osteoarthritis and the prediction of poor prognosis(P<O.05).Conclusion MRI combined with MMP-9 and IL-7R has high diagnostic value and predictive value for poor prognosis of knee osteoarthritis,which can provide reference for the diagnosis and treatment of knee osteoarthritis.
