甲状腺结节CT灌注成像参数与微血管密度及血管内皮生长因子表达的相关性研究 被引量:6

摘要 目的 :分析甲状腺结节CT灌注参数与微血管密度(MVD)、血管内皮生长因子(VEGF)表达的相关性,探讨CT灌注成像在评价甲状腺结节血管生成中的应用价值。方法:选取经术后病理证实的甲状腺结节患者41例,均采用64排螺旋CT行常规CT平扫和灌注扫描,测得血流量(BF)、血容量(BV)、平均通过时间(MTT)及表面通透性(PS)值。术后标本行CD31、VEGF抗体免疫组织化学染色,分析甲状腺结节CT灌注成像参数与MVD及VEGF表达的相关性。结果:①恶性结节的BF明显高于良性结节,MTT较良性结节短,两者差异均有统计学意义(均P<0.05),而两者BV、PS值差异均无统计学意义(均P> 0.05)。②MVD在良恶性结节间差异无统计学意义(P> 0.05),VEGF在良恶性结节间差异有统计学意义(P<0.05)。③甲状腺结节MVD、VEGF与CT各灌注参数值间无明显相关性(均P> 0.05)。结论:CT灌注成像在诊断甲状腺结节方面有一定的价值,但其术前无法反映甲状腺结节的MVD及VEGF。 Objective:To analyze the correlation between CT perfusion parameters and expression of microvessel density(MVD and vascular endothelial growth factor(VEGF) in thyroid nodules.To evaluate the value of CT perfusion imaging in the evaluation of thyroid nodule angiogenesis.Methods:Forty-one patients with thyroid nodules confirmed by pathology were examined by routine CT scan and perfusion scan with 64-slice spiral CT,and then the values of blood flow(BF),blood volume(BV),mean transit time(BTT) and permeability surface(PS) were measured.The correlation between CT perfusion imaging and MVD,VEGF expression was analyzed.Results:①The BF of malignant nodules was significantly higher than that of benign nodules and MTT was shorter than that of benign nodules(both P<0.05),but there was no significant difference between BV and PS(both P> 0.05).②There was no significant difference in MVD between benign and malignant nodules(P> 0.05).There was significant difference in the expression of VEGF between benign and malignant nodules(P <0.05).③ There was no significant correlation between MVD count and VEGF expression between benign and malignant nodules.Conclusions:CT perfusion imaging is valuable in the diagnosis of thyroid diseases.Preoperative CT perfusion imaging could not reflect the MVD and VEGF of thyroid nodules.
