摘要
目的:观察海藻玉壶汤加减联合西药治疗甲状腺结节合并亚临床甲状腺功能减退(简称甲减)的临床疗效。方法:将80例甲状腺结节合并亚临床甲减患者作为研究对象,按照随机数字表法分为治疗组和对照组,每组各40例。对照组给予常规西药治疗,治疗组在对照组的基础上给予海藻玉壶汤加减治疗。比较两组患者的临床疗效及治疗前后临床症状评分、甲状腺激素[三碘甲状腺激素(free triiodothyronine,FT3)、游离四碘甲状腺激素(free tetraodothyronine,FT4)及促甲状腺激素(thyroid stimulating hormone,TSH)]、甲状腺因子[血管内皮生长因子(vascular endothelial growth factor,VEGF)、转化生长因子-β_1(transforming growth factor-β_1,TGF-β_1)及胰岛素样生长因子-1(insulin like growth factor-1,IGF-1)]水平及超声指标[微血管密度(microvessel density,MVD)及结节大小]的变化情况。结果:治疗组有效率为90.00%,对照组有效率为67.50%,两组患者有效率比较,差异具有统计学意义(P<0.05)。两组患者治疗后临床症状积分、VEGF、IGF-I、TSH水平低于本组治疗前,且治疗组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后TGF-β_1高于本组治疗前,且治疗后治疗组高于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后MVD及结节大小小于本组治疗前,且治疗后治疗组小于对照组,差异具有统计学意义(P<0.05)。结论:海藻玉壶汤加减联合西药治疗甲状腺结节合并亚临床甲减,可改善患者临床症状、甲状腺激素水平,缩小甲状腺结节。
Objective:To observe the clinical curative effect of Modified Haizao Yuhu Decoction(海藻玉壶汤加减,MHZYHD)combined with Western medicine on thyroid nodules with subclinical hypothyroidism.Methods:A total of 80 patients with thyroid nodules with subclinical hypothyroidism were randomly divided into the treatment group and the control group,with 40 cases in each group.The control group was given conventional Western medicine treatment,and the treatment group was given MHZYHD on the basis of the control group.The clinical efficacy,the changes of clinical symptom score,thyroid hormone[including free triiodothyronine(FT3),free tetraodothyronine(FT4)and thyroid stimulating hormone(TSH)],the levels of thyroid factor[including vascular endothelial growth factor(VEGF),transforming growth factor-β1(transforming growth factor-β1,TGF-β1),insulin-like growth factor-1(IGF-1)]before and after treatment in both groups were measured.Results:The effective rate of the treatment group was 90.00%,and that of the control group was 67.50%.The difference between the two groups was statistically significant(P<0.05).After treatment,the clinical symptom score,VEGF,IGF-I,TSH levels of the two groups were lower than those of the same group before treatment,and the above indexes of the treatment group were lower than those of the control group,and all the differences were statistically significant(P<0.05).TGF-β1 of the two groups after treatment was higher than that of the same group before treatment the treatment group′s was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,MVD and nodule diameter of the two groups were smaller than those before treatment,and the above indexes of the treatment group were smaller than those of the control group,and all the differences were statistically significant(P<0.05).Conclusion:In terms of thyroid nodules with subclinical hypothyroidism,MHZYHD combined with Western medicine can improve the clinical symptoms,thyroid hormone levels
