摘要
目的评价人附睾蛋白-4(HE4)鉴别良恶性胸腔积液的参考价值。方法搜索CNKI、万方医学网、维普数据库、PubMed、Cochrane等数据库,收集有关HE4与良恶性胸腔积液的相关文献,进行信息统计、质量分级、异质性检验,运用Meta-Disc1.4软件进行数据分析。结果共纳入5篇文献,恶性胸腔积液组276例,良性胸腔积液组301例,通过随机效应模型的汇总得出HE4在恶性胸腔积液诊断中的灵敏度和特异度分别为0.78(95%CI 0.72~0.82)和0.89(95%CI 0.85~0.92);阳性似然比和阴性似然比分别为6.62(95%CI 4.79~9.16)和0.23(95%CI 0.13~0.39);诊断比值比为31.60(95%CI 13.84~72.15)。结论基于现有的循证医学证据,胸水中HE4可成为良恶性胸腔积液鉴别的参考指标,当作临床诊断恶性胸腔积液的定量检测指标。
Objective To evaluate the reference value of human epididymis protein-4(HE4) in differentiating benign and malignant pleural effusion. Methods CNKI, Wanfang Medical Network, Weipu database, PubMed, Cochrane database were searched and related literatures about HE4 and benign and malignant pleural effusion were collected. Information statistics, quality grading and heterogeneity test were carried out. Meta-Disc 1.4 software was used for data analysis.Results A total of 5 literatures were included, in which there were 276 cases in malignant pleural effusion group and 301 cases in benign pleural effusion group. The sensitivity and specificity of HE4 in the diagnosis of malignant pleural effusion were 0.78(95% CI 0.72-0.82) and 0.89(95% CI 0.85-0.92), respectively. The positive likelihood ratio and negative likelihood ratio were 6.62(95%CI 4.79-9.16) and 0.23(95%CI 0.13-0.39), respectively. The diagnostic ratio was 31.60(95%CI 13.84-72.15). Conclusion Based on the existing evidence-based medicine, HE4 in pleural effusion can be used as a reference index for differentiating benign and malignant pleural effusion and as a quantitative detection index for clinical diagnosis of malignant pleural effusion.
