摘要
目的探讨活性氧物质调节因子1(reactive oxygen species modulator 1, Romo1)和癌胚抗原(carcinoembryonic antigen, CEA)对恶性胸腔积液(malignant pleural effusion, MPE)的诊断价值。方法收集48例MPE和26例良性胸腔积液(benign pleural effusion, BPE)标本,应用酶联免疫吸附测定法测定Romo1和CEA在胸腔积液中的表达量,根据测得数据绘制受试者工作曲线(ROC曲线)及联合诊断ROC曲线,计算相应曲线下面积(AUC)、诊断界值、敏感度、特异度。结果 Romo1诊断MPE的AUC为0.795,相应敏感度、特异度为60.4%、88.5%;CEA的AUC为0.851,相应敏感度、特异度为64.6%、93.6%;二者联合诊断的AUC为0.916,敏感度、特异度为72.9%、96.2%;P 均<0.001。结论 Romo1和CEA均可作为MPE的诊断指标,二者联合应用具有更高的敏感度和特异度,有望在临床中广泛应用。
Objective To investigate the diagnostic value of reactive oxygen species modulator 1 (Romo1) and carcinoembryonic antigen (CEA) in malignant pleural effusion (MPE). Methods The samples were collected from 48 MPE cases and 26 benign pleural effusion (BPE). The enzyme-linked immunosorbent assay (ELISA) was used to detect the expression of Romo1 and CEA in the pleural effusion, and the collected data were used to draw the receiver operator characteristic (ROC) curve and the joint diagnostic ROC curve. The area under curve (AUC), diagnostic threshold, sensitivity and specificity were calculated. Results The AUC of Romo1 was 0.795 with the sensitivity of 60.4% and the specificity of 88.5% and the AUC of CEA was 0.851 with the sensitivity of 64.6% and the specificity of 93.6% in diagnosing MPE. The AUC of the joint diagnosis was 0.916 with the sensitivity of 72.9% and the specificity of 96.2%. Conclusion Romo1 and CEA can be used as the diagnostic indicator of MPE. The joint diagnosis of Romo1 and CEA is of higher sensitivity and specificity, so it is expected to be widely used in the future clinical practice.
