摘要
目的:分析中国胃癌发病率30年来的变化趋势,找到影响胃癌发病率的最主要原因,为中国胃癌的防治提供有用的指导意见。方法:收集各影响因素的历年变化趋势数据,采用双变量相关的统计学方法,与当年胃癌的发病率进行相关性分析,求出Spearman等级相关系数r_s,并对有统计学意义的影响因素按照r_s绝对值的大小进行排序。结果:农村胃癌的发病率总体呈下降趋势,而城市胃癌的发病率一直趋于平稳,且远低于农村的水平,但从2009年开始小幅上升,使得城乡胃癌发病率水平基本接近。与城乡胃癌发病率负相关的有:新鲜瓜果消费量、可支配收入、电冰箱拥有量、老龄化水平;仅与城市胃癌发病率负相关的有:城市人口比例;仅与农村胃癌发病率正相关的有:高盐腌渍食物消费量、工业废水化学需氧量排放量、工业废水氨氮排放量、农村人口比例。结论:我国近30年来胃癌发病率呈逐渐下降趋势,其主要原因在于社会经济的发展和生活方式的改变。
Objective: To analyze the trend of gastric cancer incidence in China during 30 years, find the most important factor affecting the incidence of gastric cancer, and provide useful guidance for the prevention and treatment of gastric cancer in China. Methods: Weanalyzed the correlation between each influencing factor and the incidence of gastric cancer in that year.Firstly, we recorded each influencing factor and figured out rs, the Spearman rank correlation coefficient, using bivariate correlation statistical method. Secondly, we ordered the statistically significant influential factors by the absolute values of r_s. Results: The incidence of gastric cancer in rural areas declined generally, while that in urban areas had been stable and far below the level in rural areas. However, the incidences of gastric cancer in urban and rural areas became close since 2009 when the incidence in urban areasbegan to rise slightly. The incidence of gastric cancer was negatively correlated tofresh fruit consumption, disposable income, refrigerator ownership and the aging level.;that in urban areas was negatively correlated to the proportion of urban population;and that in rural areaswas positively correlated tothe consumption of highly salted food, the proportion of chemical oxygen demand in industrial wastewater, the proportion of ammonia nitrogen in industrial wastewater and the proportion of rural population. Conclusion: In recent 30 years, the incidence of gastric cancer has been decreasing in China, which is mainly caused by the development of social economy and the change of life style.
