摘要
目的:观察不同营养不良程度及其炎症状态对老年病人肠屏障功能的影响,为早期干预提供依据。方法:对老年病人依据营养状况分组,记录一般情况、生化指标、肠屏障指标:二胺氧化酶(DAO)、D-乳酸(D-Lac)、内毒素(LPS)浓度;比较不同分组及其炎症状态下的肠屏障功能。结果:研究纳入75例病人,无炎症状态下营养正常组、营养风险组、营养不良组肠屏障指标无明显统计学差异(P>0.05),炎症状态下三组肠屏障指标均较前显著升高(P<0.01);各组肠屏障指标前后差值(D值)相比较,营养不良组的DAO-D值较营养风险组显著增高,营养风险组的DAO-D值、D-Lac-D值较营养正常组显著增高,差异具有显著统计学意义(P<0.01)。结论:伴随营养状况恶化,炎症对老年病人肠屏障的损伤程度也进一步加剧,其中DAO可动态反映损伤程度,可作为监测指标。
Objective:To observe the effects of malnourished degrees and inflammatory state of elderly patients on the intestinal barrier function,so as to provide evidence for early intervention.Methods:Elderly patients were grouped according to nutritional status.The intestinal barrier function was reflected by levels of diamine oxidase(DAO),D-lactic acid(D-Lac),and endotoxin(LPS),which were determined under different inflammatory condition.Results:75 patients were enrolled in the study(P>0.05).There were no obvious differences on the intestinal barrier indexes among the three groups without inflammation.Under inflammatory condition,levels of intestinal barrier indexes of all patient were significantly higher than normal condition(P<0.01);the level of DAO-D of the malnutrition group was significantly higher than that of the nutritional risk group.The DAO-D and D-Lac-D values of the nutritional risk group were significantly higher than those of the normal nutritional group(P<0.01).Conclusion:With the deterioration of nutritional status of elderly patients,the degree of damage by inflammation to the intestinal barrier is further aggravated.DAO can reflect the degree of damage dynamically and can be used as a monitoring indicator of intestinal barrier injury.
