智能控温冷敷仪在骨折创伤患者中不同应用时间的对比研究 被引量:6

Comparative study on the effects of the application of smart temperature-controlled cold compressing device on the fracture patients induced by trauma at different time points

摘要 目的探讨骨折创伤患者应用自主研发的可控温智能冷敷仪冷敷的最佳时间。方法选择2015年12月至2016年12月于南京医科大学附属南京医院骨科就诊的骨折创伤患者77例,按照入院先后顺序进行编号,随机分为4组,使用可控温智能冷敷仪保持20℃恒定温度冷敷创伤处,分别持续冷敷1、6、24、72 h。冷敷前后,测量患肢周径,评估疼痛;调查患者舒适度及护士工作满意度。结果冷敷前4组患肢肿胀程度无差异(P>0.05);冷敷后,4组患肢肿胀周径差值比较差异有统计学意义(F=2.886,P=0.041);组间对比,冷敷24 h组显著优于冷敷1 h组(P=0.005);而其他组两两比较差异无统计学意义(P>0.05)。冷敷后,4组患者疼痛缓解程度比较差异有统计学意义(F=17.379,P<0.001);组间比较,冷敷24、72 h组分别较冷敷1、6 h组疼痛程度明显减轻,均P<0.001。4组患者舒适度比较差异无统计学意义(P>0.05)。4组护士工作满意度比较差异具有统计学意义(P<0.05),其中冷敷24 h组较冷敷1、6、72 h组满意度高(P<0.05)。4组患者骨折愈合时间比较差异有统计学意义(P<0.05)。冷敷24、72 h组较冷敷1、6 h组愈合快(P<0.05)。结论骨折创伤患者最佳冷敷时间为24 h。智能控温冷敷骨折患者能有效消肿止痛,促进骨折愈合,不影响患者舒适度,提高护士工作满意度。 Objective To explore the optimal time of the application of self-made smart temperature-controlled cold compressing device for traumatic fracture patients.Methods Seventy-seven traumatic fracture patients admitted into the hospital from December, 2015 to December, 2016 were enrolled for the study. According to the sequence of admission, they were divided into 4 groups, i.e. group A, B, C, and D. The smart temperature-controlled cold compressing device was applied to the injury site with a fixed constant temperature of 20 ℃ for cold-compression for respectively 1, 6, 24 and 72 hours. Before and after cold compression, the circumference of the affected limb and seriousness of pain were detected, and the rates of comfort and satisfaction with the work of the nurses were investigated in the study.Results No differences could be seen in the seriousness of swelling in the affected limbs before cold compression(P>0.05). After cold compression, differences could be noted in the circumferences of the affected limbs in the 4 groups, and statistical significance could be noticed when comparisons were made between the 4 groups(F=2.886, P=0.041).The 24-h group(or group C) was significantly better than the 1-hour group(or group A)(P=0.005). However, no statistical significance could be seen, when it was compared with the 2 other groups(P>0.05). After cold compression, there was statistical significance in the seriousness of pain, when comparisons were made between the 4 groups(F=17.379, P<0.001). The seriousness of pain for the 24-h and 72-h groups was obviously alleviated, when it was compared with that of the 1-h and 6-h groups(P<0.001). There was no statistical significance in the rate of comfort, when comparisons were made between the 4 groups(P>0.05). However, there was statistical significance in patient satisfaction with the nurse work, when comparisons were made between the 4 groups, and the rate of satisfaction for the 24-h group was higher than those of the 1-h, 6-h and 72-h groups(P<0.05).
