摘要
目的探讨在踝关节骨折围手术期局部冷敷的应用效果。方法选择本院骨科2011年1月-2015年1月收治的踝关节骨折患者90例,按照入院时间段分为研究组和对照组各45例。对照组骨折后采用常规处理,并静脉滴注七叶皂苷钠;研究组在常规处理后,采用奥克兰冷敷治疗器进行局部冷敷治疗,保持皮肤温度在10-15℃,持续冷敷36 h。分别在骨折后12、24、48 h,对患者疼痛程度进行VAS评分;骨折后1、2、3 d,分别测量患者内踝尖水平周径;记录患者术前等待时间。结果与对照组比较,骨折后12、24、48 h,研究组疼痛程度明显减轻（P〈0.05）;在骨折后1、2、3 d,研究组内踝尖水平周径明显小于对照组（P〈0.05）;研究组术前等待时间为（4.2±2.2）d,较对照组的（8.6±2.8）d明显缩短（P〈0.05）。结论局部冷敷能有效缓解踝关节骨折患者的疼痛,有助于踝关节快速消肿,并能缩短手术等待时间。
Objective To explore the application effect of local cold compress on ankle joint fracture during perioperative period.Methods A total of 90 patients who had been suffering ankle joint fracture and had been admitted to the Department of Orthopaedics of our hospital from January 2011 to January 2015 were selected and divided into a study group and a control group（n=45,respectively） according to the admission time.The control group received conventional treatment and intravenously injected with sodium aescinate upon fracture; the study group received,upon conventional treatment,local cold compress with Auckland cold compress therapy equipment to keep the skin temperature at 10-15 ℃ for 36 h.The pain degree was scored with VAS scores respectively 12,24 and 48 h after the fracture; the cross-section diameter of the prominence of the lateral malleolus was measured respectively on the first,second and third day upon the fracture; the preoperative waiting time was recorded.Results Compared with the control group,the pain degree of the study group was obviously relieved 12,24 and 48 h after the fracture（P〈0.05）; the crosssection diameter of the study group on the first,second and third day upon the fracture was much smaller that of the control group（P〈0.05）; the preoperative waiting time of the study group was（4.2±2.2）d,which was much shorter than the（8.6±2.8）d of the control group（P〈0.05）.Conclusion Local cold compress can effectively relieve the pains of patients with ankle joint fracture,accelerate the detumescence of the ankle joint,and shorten the preoperative waiting time.
