摘要
目的观察活血消肿汤联合西医常规治疗对肱骨近端骨折术后的临床疗效。方法将80例肱骨近端骨折术后患者按照随机数字法分为2组,对照组40例予西医常规治疗,治疗组40例在对照组基础上联合活血消肿汤治疗。2组均治疗8周后统计疗效,比较2组治疗前后肩关节患侧周径及健患侧皮温差变化情况,比较2组治疗前及治疗1周后肩关节疼痛视觉模拟评分(VAS)变化情况,比较2组治疗前后Constant-Murley肩关节功能评分量表(CMS)(包括关节活动度、三角肌肌力、疼痛及日常生活能力)评分变化情况,比较2组治疗前后患肢肿胀程度改善情况,比较2组治疗前后血清C反应蛋白(CRP)和肌酸激酶(CK)水平变化情况。结果治疗组总有效率95.0%(38/40),患肢肿胀程度总改善率92.5%(37/40),对照组分别为80.0%(32/40)、72.5%(29/40),治疗组总改善率及患肢肿胀程度总改善率均高于对照组(P<0.05)。与本组治疗前比较,2组治疗后肩关节患侧周径及健患侧皮温差均降低(P<0.05),且治疗组治疗后肩关节患侧周径及健患侧皮温差均低于对照组(P<0.05)。与本组治疗前比较,2组治疗1周后疼痛VAS均降低(P<0.05),且治疗组治疗1周后疼痛VAS低于对照组(P<0.05)。与本组治疗前比较,2组治疗后CMS关节活动度、三角肌肌力、疼痛、日常生活能力评分及总分均升高(P<0.05),且治疗组治疗后CMS关节活动度、三角肌肌力、疼痛、日常生活能力评分及总分均高于对照组(P<0.05)。与本组治疗前比较,2组治疗后血清CRP及CK水平均降低(P<0.05),且治疗组治疗后血清CRP及CK水平均低于对照组(P<0.05)。结论活血消肿汤联合西医常规治疗肱骨近端骨折术后临床疗效确切,可明显促进术后肿胀消退,缓解疼痛,促进肩关节功能恢复,抑制CRP、CK表达,减轻炎性反应,从而促进肱骨近端骨折术后康复,且安全可靠。
Objective To observe the clinical efficacy of Huoxue Xiaozhong Decoction combined with conventional Western medicine for proximal humerus fracture(PHF).Methods Totally 80 PHF patients were randomized(1:1)to receive conventional Western medicine(the control group)or Huoxue Xiaozhong Decoction+conventional Western medicine(the treatment group)for 8 weeks.The aiming was to compare perimeter and skin temperature of shoulder joint,visual analogue scale(VAS)for shoulder pain on day 0,and on week 1,Constant-Murley Score(CMS)(joint motion,deltoid muscle strength,pain,activity of daily living[ADL]),improvement of limb swelling degree,C-reactive protein(CRP),creatine kinase(CK);the curative effect was assessed.Results The overall effective rate and the total improvement rate of limb swelling degree in the treatment group were higher than those in the control group(95.0%[38/40]vs 80.0%[32/40],92.5%[37/40]vs 72.5%[29/40],[P<0.05],respectively).After treatment,the perimeter and skin temperature of shoulder joint in the both groups were decreased(P<0.05),which were lower in the treatment group than in the control group(P<0.05);1-week VAS in the both groups was decreased(P<0.05),which was decreased in the treatment group relative to the control group(P<0.05);CMS in the both groups was increased(P<0.05),which in the treatment group was more prevalent with respect to the control group(P<0.05);CRP and CK in the both groups were decreased(P<0.05),the reduction in the treatment group was more common(P<0.05).Conclusion For PHF patients,Huoxue Xiaozhong Decoction combined with conventional Western medicine has definite clinical curative effect,which can significantly reduce postoperative swelling,relieve pain,promote the recovery of shoulder joint function,inhibit the expression of CRP and CK,reduce inflammatory reactions,thereby promoting postoperative rehabilitation,and with safety and reliability.
