摘要
目的探讨主动脉窦内室性早搏(简称室早)零射线射频消融的可行性并与传统X线指导下消融方法进行比较。方法随机选取2015年11月至2016年11月收治的主动脉窦内室早患者67例,依据不同操作方法分为两组,33例于X线指导下行射频消融治疗者为对照组,34例于Carto3系统重建下并采用体表以及电位定位法,行零射线射频消融治疗者为试验组,对比分析两组即时成功率、手术指标、并发症发生情况。结果试验组即刻成功率、远期成功率较对照组高(100%vs 88%;97%vs 82%,P<0.05);与对照组比较,试验组X线透视时间为零,标测用时更短,放电次数更少[(34.9±2.3)min vs(59.0±4.3)min,(4.6±0.8)次vs(9.5±1.2)次,P<0.05];并发症发生率也比对照组低。结论零射线射频消融术治疗主动脉窦内室早可提升临床效果及安全性。
Objective To investigate the methods and effects of zero-fluoroscopy radiofrequency ablation of the premature ventricular eontraction(PVC) in the aortic sinus. Methods Sixty-seven patients with PVC were randomly selected in our hospital from November 2015 to November 2017.According to the different treatment methods,they were divided into two groups, 33 cases under the direction of X-ray exposure as the control group, 34 cases without X-ray and under the direction of Carto3 3D mapping technique with surface location and potential location methods as the experimental group. Their clinic data were comparatively analyzed in instant success rates, index of operation, complications and recurrence. Results The instant success rates and the long-term success rates in the experimental group were higher than that in the control group, with statistically significant ((100 % vs 88 % ; 97 vs 82 %, P〈 0.05) ; except of zero exposure, mapping time, discharge times in the experimental group were shorter than those in the control group, with statistically significant [(34.9±2.3) min vs (59.0±4.3)min, (4.6±0.8)times vs (9.5±1.2)times,P〈0.05] ; the experimental group has less complications compared with the control group. Conclusion Zero-fluoroscopy radiofrequeney ablation methods in the treatment of PVC in the aortic sinus can improve the clinical effect and safety.
