摘要
目的探讨室性早搏后窦性搏动ST-T改变的临床分析。方法选择2016年12月至2018年12月本院收治的室性早搏后窦性搏动ST-T改变患者236例作为研究对象,分析不同波形改变导致的疾病发生率、波形改变的因素以及预后情况。结果236例中,T波改变198(83.90%)例,其中出现无器质性心脏病147例,冠心病23例,心肌炎5例,心肌病14例,高血压性心脏病9例;ST段改变16(6.78%)例,全部为器质性心脏病,其中冠心病9例,心肌炎2例,高血压性心脏病5例;ST-T同时改变22(9.32%)例,全部为器质性心脏病,其中冠心病10例,心肌病4例,高血压性心脏病8例。T波改变中无器质性心脏病发病率高于器质性心脏病(P<0.05);ST段、ST-T同时改变表明器质性心脏病发生,其中冠心病发生率高于其他疾病(P<0.05)。多因素Logistic分析结果表明,ST-T改变与性别、是否伴有高血压显著相关(P<0.05)。ST-T有改变的配对间期越短,早搏后窦性搏动越易出现ST-T改变(P<0.05)。结论室性早搏后窦性搏动ST-T改变可以作为部分心脏类疾病的诊断依据,ST-T的改变与性别和高血压有关,且与室性早搏发生的时间有关。
Objective To investigate the clinical analysis of sinus beat ST-T changes after ventricular premature beats.Methods 236 ST-T changes in sinus beats after ventricular premature beats were performed from December 2016 to December 2018 in our hospital.The incidence of disease changes and the prognosis of different corrugations were analyzed.Results Among 236 cases,T segment was changed by 198(83.90%)cases,including 147 cases of no structural heart disease,23 cases of coronary heart disease,5 cases of myocarditis,14 cases of cardiomyopathy,9 cases of hypertensive heart disease.ST Segment change 16(6.78%)cases,all of which were organic heart disease,including 9 cases of coronary heart disease,2 cases of myocarditis,5 cases of hypertensive heart disease.ST-T changed 22(9.32%)cases at the same time,all of them were quality heart disease,including 10 cases of coronary heart disease,4 cases of cardiomyopathy,8 cases of hypertensive heart disease.The incidence of no structural heart disease in T-segment changes was higher than that in organic heart disease(P<0.05).The simultaneous changes of ST segment and ST-T indicated that organic heart disease occurred,and the incidence of coronary heart disease was higher than other diseases(P<0.05).Multivariate Logistic analysis showed that ST-T changes and gender were significant associated with hypertension(P<0.05).The shorter the mating interval of ST-T changes,the more likely the sinus beat after premature beats to change ST-T(P<0.05).Conclusion ST-T changes in sinus beats after ventricular premature beats can be used as basis for the diagnosis of some heart diseases.ST-T changes are related to gender and hypertension,and are related to the timing of ventricular premature beats.
