摘要
目的探讨常规放疗和三维适形放疗在治疗非小细胞肺癌时引起肺损伤的相关因素。方法将研究对象按照随机分组法分为常规放疗组(51例)和三维适形放疗组(62例),并分析常规放疗以及三维适形放疗与放射性肺炎的相关性。
结果常规放疗组中照射面积100 cm2以下和100 cm2以上放射性肺炎发生率分别为8.6%和31.3%,差异有统计学意义(P〈0.05);照射剂量低于70 Gy和高于70 Gy时发生率分别为13.3%和66.7%,差异有统计学意义(P〈0.05)。三维适放射治疗组中单次照射剂量低于5 Gy和超过6 Gy时放射性肺炎发生率分别为6.7%和25.0%,差异有统计学意义(P〈0.05)。结论非小细胞肺癌患者放射治疗时,常规放射治疗与放射性肺炎相关的影响因素为照射总剂量和面积,而三维适形放疗与放射性肺炎单次剂量大小有关。
ObjectiveTo investigate the related factors of dimensional conformal radiotherapy and conventional radiotherapy-induced lung injury in patients with non-small cell lung cancer.MethodsThe objects were randomly divided into conventional radiotherapy group(51 cases) and three-dimensional conformal radiotherapy group(62 cases). The relationship between conventional radiotherapy, three-dimensional conformal radiotherapy and radiation pneumonitis was analyzed.ResultsIn conventional radiotherapy group, the incidence of radiation pneumonitis was 8.6% in patients with radiation area below 100 cm2 and 31.3% in patients with radiation area above 100 cm2, the difference was significant (P〈0.05); the incidence of radiation pneumonitis was 13.3% in patients with radiation dose below 70 Gy and 66.7 % in patients with radiation dose above 70 Gy, the difference was significant (P〈0.05). In three-dimensional conformal radiotherapy group, the incidence of radiation pneumonitis was 6.7% in patients with single radiation dose below 5 Gy and 25.0% in patients with single radiation dose above 6 Gy, the difference was significant (P〈0.05).ConclusionsWhen non-small cell lung cancer with radiation therapy, the related factors of conventional radiation therapy and radiation pneumonitis are total radiation dose and the area, and the related factors of three-dimensional conformal radiotherapy and radiation pneumonitis is single radiation dose.
