摘要
目的探讨OCM微创小切口全髋关节置换术治疗老年股骨颈骨折的临床疗效。方法选取2014年6月~2016年2月于本院接受髋关节置换手术的26例老年股骨颈骨折患者作为研究对象,患者均通过前外侧OCM入路进行手术治疗,通过观察记录手术切口长度、手术时间、术中出血情况、术后VAS评分等指标对患者临床效果进行评价。结果入组患者术后当天即可进行主动屈髋锻炼,术后2~3 d可站立或行走;患者平均手术切口为（8.4±0.5）cm,平均手术时间为（80±11）min;平均术中出血量为（221±69）ml;术后3 d的VAS评分为（1.9±1.0）,患者均病情稳定,恢复良好。结论采用前外侧OCM入路小切口全髋关节置换术治疗老年股骨颈骨折具有良好的临床疗效,手术切口小,出血量少、手术时间短,患者疼痛较低,是一种有效的临床治疗手段。
Objective To explore the clinical effect of total hip arthroplasty with OCM minimal invasive technique in the treatment of femoral neck fractures in elderly patients. Methods 26 cases of femoral neck fractures from June 2014 to February 2016 were selected and by OCM anterolateral approach. The operative incision lengths, operation time, the intraoperative hemorrhage, VAS score and clinical effect were evaluated. Results The cases could do some flexionon training on the right postoperative day, and could stand and walk on the 2~3 d. The average operative incision lengths was（8.4±0.5） cm, the average operation time was（80±11） min, the average intraoperative hemorrhage was（221±69） ml and the VAS score in 3 d was（1.9±1.0）. All patients in stable condition and had a good recovery. Conclusion Total hip arthroplasty with OCM minimal invasive technique in the treatment of femoral neck fractures in elderly patients with shorter operative incision lengths, less intraoperative hemorrhage, shorter time and lower pain levels.
